Abstract

Acne vulgaris merupakan kulit yang mengalami peradangan kronik yang ditandai dengan adanya lesi. Pengobatan acne vulgaris didasarkan pada kombinasi terapi topikal, perawatan sistemik, chemical peeling, dermabrasi, dan laser. Baru-baru ini, prosedur chemical peeling lebih umum digunakan acne vulgaris, dan acne scar. Penelitian-penelitian terdahulu menunjukkan jika chemical peeling digunakan efektif acne vulgaris ringan sampai sedang dan ditoleransi dengan baik. Namun tidak dapat ditarik kesimpulan mengenai keunggulan atau kesetaraan definitif di antara pengelupasan kimia yang saat ini digunakan. Penelitian ini bertujuan untuk mengetahui efektivitas penggunaan superficial chemical peeling (SCP) terhadap acne vulgaris (mild-moderate) dalam penurunan jumlah lesi total, perbaikan hiperpigmentasi pasca jerawat dan perbaikan dari bekas jerawat (acne scars). Desain penelitian yang digunakan adalah Systematic Literature Review. Data sekunder yang digunakan diperoleh melalui database online seperti Google scholar, Crossref, Scopus, Pubmed (2015-2021). Total hasil pencarian yang didapat dari 4 database tersebut yang sesuai dengan kriteria inklusi yaitu sebanyak 11 artikel. Dari 11 artikel yang terpilih, efektivitas SCP tidak menunjukkan perbedaan yang signifikan. Presentase dalam menurunkan jumlah lesi 32,62% - 95% (asam azelaic, asam salisilat (SA), Trichloroacetic acid, Jessner’s solution (JS), asam glikolat, asam piruvat) perbaikan hiperpigmentasi pasca-jerawat, PAHPI score: 11 (JS) vs 10 (SA), p=0.662, perbaikan acne scars pada bagian superfisial. Selain itu kombinasi asam salisilat dengan asam mandelic lebih efektif dibandingkan asam glikolat dalam pengobatan kedua non-inflamasi dan lesi jerawat inflamasi 66,13% vs 46,88%, p=0.001. Kesimpulannya asam salisilat 30% (solution) efektif dalam menurunkan jumlah lesi (95%), memperbaiki hiperpigmentasi pasca-jerawat (PAHPI score:10), menurunkan bekas jerawat / acne scars sampai batas tertentu.

English Abstract

Acne vulgaris is a chronic inflammatory skin characterized by lesions. Treatment of acne vulgaris is based on combination of topical therapies, systemic treatments, chemical peels, dermabrasion, and lasers. Recently, chemical peeling procedures are more commonly used in acne vulgaris, and acne scars. Previous studies have shown that chemical peels are effective in mild to moderate acne vulgaris and are well tolerated. However, no conclusions can be drawn regarding the superiority or definitive equivalence among the chemical peels currently used. This study aims to determine the effectiveness of superficial chemical peels usage against acne vulgaris (mild-moderate) in reducing the total number of lesions, improving post-acne hyperpigmentation and improving acne scars. The research design was carried out by the Systematic Literature Review. The secondary data used in the research were obtained from online databases such as Google scholar, Crossref, Scopus, Pubmed (2015-2021). The total of search results that were gained from those 4 databases which matched the inclusion criteria were 11 articles. There was no significant difference in the effectiveness of superficial chemical peeling from the 11 selected articles. The percentage of number of lesions reduction was 32.62% - 95% (azelaic acid, salicylic acid (SA), Trichloroacetic acid, Jessner's solution (JS), glycolic acid, pyruvic acid), and the PAHPI score of post-acne hyperpigmentation recovery was 11 (JS) vs 10 (SA), (p=0.662). Based on those reviews, salicylic acid (solution) was 30% effective in reducing the number of lesions (95%), reforming post-acne hyperpigmentation (PAHPI score:10), and reducing the acne scars to some extent.
