Abstract

Hipertensi ditemukan pada lebih dari 50% pasien diabetes mellitus. Tekanan darah yang tinggi adalah salah satu faktor risiko terjadinya penyakit jantung koroner dan stroke. Dari penelitian terdahulu diketahui peningkatan ketebalan tunika intima-media dapat meningkatkan risiko penyakit kardiovaskular. Tujuan dari penelitian ini ialah untuk mengetahui berapa besar pengaruh hipertensi terhadap ketebalan tunika intima-media arteri karotis pada pasien diabetes mellitus. Penelitian ini dilakukan menggunakan metode observasional analitik dengan pendekatan cross-sectional melalui pengambilan data tekanan darah dan pengukuran ketebalan arteri karotis dengan USG Doppler di RS Saiful Anwar Malang. Pemilihan subjek penelitian menggunakan purposive sampling dan didapatkan subjek sebanyak 34 orang, 8 orang dieksklusi karena tidak sesuai dengan kriteria inklusi. Hasil uji Saphiro-Wilk mengatakan data terdistribusi secara normal karena dengan p = 0.385. Uji asumsi multikolineritas didapatkan nilai toleransi 0.790 dan VIF 1.265 sehingga tidak terjadi gejala multikoliner. Hasil uji R Square a. karotis sinistra diketahui sebesar 0.244% menandakan tekanan darah berpengaruh sebesar 24.4% pada ketebalan a. karotis sinistra. Pada a. karotis dextra didapatkan R Square sebesar 0.123, menandakan tekanan darah berpengaruh sebesar 12.3% pada a. karotis dextra. Regresi linear berganda pada a. karotis sinistra didapatkan konstanta 0.482, koefisien sistolik 0.004 dan koefisien diastolik -0.005. Hasil regresi linear berganda pada a. karotis dextra menunjukan konstanta 0.682, koefisien sistolik 0.003 dan koefisien diastolik -0.005. Berdasarkan hasil analisis diketahui tekanan darah sistolik berpengaruh lebih besar pada ketebalan tunika intima-media sehingga peningkatan tekanan sistolik lebih berisiko terjadi aterosklerosis, namun hasil analisis tekanan darah diastolik tidak berpengaruh secara langsung pada ketebalan tunika intima-media arteri karotis. Perlu dilakukan penelitian dengan mempertimbangkan faktor risiko lain dan jumlah subjek yang lebih banyak.

English Abstract

High blood pressure was found in more than 50% diabetes mellitus patient. High blood pressure is known to be a risk factor of coronary heart disease and stroke. From previous study known that the rise in intima-media thickness is propotional to an increase in cardiovascular risk factor. The purpose of this study was to find out how much impact hypertension has on intima-media thickness in diabetes mellitus patient. The method used in this study was analytic observational with a cross-sectional approach through measurement of the blood pressure and intima-media thickness using Doppler Ultrasonography in Saiful Anwar Hospital Malang. By using purposive sampling, there were 34 patients as research sample, 8 patients being excluded because not suitable with the inclusion criteria. Saphiro-Wilk test showed that the data was normally distribute (p = 0.385). From multicollinearity assumption test was obtained tolerance value 0.790 and VIF 1.265, this indicates a negative multicollinearity. R Square from sinistra carotid artery showed 0.244% value, means blood pressure had 24.4% effect on sinistra carotid artery thickness. On dextra carotid artery R Square showed 0.123 value, means blood pressure had 1.23% effect on dextra carotid artery thickness. Constanta 0.482, systolic coefficient 0.004, and diastolik coefficient -0.005 was known from multiple linear regression test in sinsitra carotid artery. The results from dextra carotid artery showed constanta 0.682, systolic coefficient 0.003, and diastolik coefficient -0.005. So it can be conclude that systolic blood pressure have a larger impact on intima-media thickness so in an increase thickness is more vulnerable to atherosclerosis, but the impact of diastolic blood pressure is not accordance with the hypothesis. Future study is needed with a consideration of other risk factors and extra amount of sample.
