Abstract
Diabetes melitus tipe 2 (non-insulin dependent DM) terjadi karena adanya resistensi insulin dan disfungsi sel β pankreas. Rokok dengan kandungan nikotinnya mampu mengurangi efektivitas dari hormon insulin dalam regulasi glukosa darah pada penderita diabetes mellitus tipe 2. Perokok aktif dan pasif mampu menyebabkan penebalan dinding arteri dan mampu merubah fungsi endothelial pembuluh darah. Penebalan tunika intima-media arteri karotis (CIMT) meningkatkan risiko terjadinya aterosklerosis dan stroke. Ultrasonografi Doppler (USG) adalah alat untuk mengevaluasi CIMT. Penelitian ini bertujuan untuk mengetahui pengaruh perilaku merokok terhadap ketebalan tunika intima-media arteri karotis pada pasien diabetes melitus. Penelitian ini menggunakan metode analisis observasional dengan metode rancangan cross sectional. Sampel yang digunakan adalah 34 responden penderita diabetes melitus tipe 2. Hasil uji beda didapatkan perbedaan yang tidak bermakna antara perilaku merokok terhadap CIMT baik dextra maupun sinistra pada pasien diabetes melitus tipe 2 (p = 0.889 dan p = 0.161). Hasil uji hubungan spearman test, didapatkan hubungan berarah positif antara usia dengan CIMT baik yang merokok maupun tidak merokok (p = 0.59 dan p = 0.41). Dan untuk uji hubungan chi-square test, melihat hubungan jenis kelamin dan CIMT baik yang merokok dan tidak merokok (p = 0.25 dan p = 0.07). Dapat disimpulkan perilaku merokok terhadap CIMT dextra ataupun sinistra tidak memiliki nilai perbedaan yang bermakna. Sedangkan untuk usia dan jenis kelamin terhadap CIMT merokok ataupun tidak merokok tidak memiliki hubungan yang bermakna.
English Abstract
Type 2 diabetes mellitus (non-insulin dependent DM) happens due to insulin resistance and dysfunction of β pancreas cell. Cigarette with nicotine as one of the chemical substances may reduce the effectivity of insulin as a regulator of blood glucose in type 2 diabetic patients. Active and passive smoker can induce the thickening of arterial wall and alter the normal endothelial function. Thickening of carotid arterial tunica intima-media (CIMT) might increase risk of atherosclerotic and stroke. Doppler ultrasonography (USG) is one of the way to evaluate CIMT. The main aim of this research is to understand the effect of smoking behavior towards the thickness of tunika intima-media of carotid artery in diabetic patients. This research uses a cross-sectional observational analytical method. There were 34 respondents type 2 diabetic patients. There were no significant difference in smoking behavior towards CIMT dextra and sinistra of patients (p = 0.889 dan p = 0.161). There were positive correlation between age and CIMT in both smokers and non-smokers (p = 0.59 and p = 0.41). There are no correlation found between sex and CIMT in both smokers and non-smokers (p=0.25 and p=0.07) in chi-square analysis. There are no significant differences between smoking behavior on dextra and sinistra CIMT. There are no significant correlation between sex and age towards smokers and non-smokers CIMT.
