Abstract

Indonesia masuk dalam kategori 30 negara dengan beban Tuberkulosis tertinggi di dunia karena terdapat baik kasus Tuberkulosis, ko-infeksi TBC-HIV, dan Tuberkulosis resisten obat. Tuberkulosis pada anak kurang mendapatkan perhatian dibandingkan orang dewasa karena sulitnya diagnosis. Salah satu metode diagnosis Tuberkulosis yang mulai digunakan adalah IGRA akan tetapi penelitian untuk mengetahui nilai diagnostik pemeriksaan IGRA pada anak belum banyak dilaporkan. Penelitian ini bertujuan untuk mengetahui nilai diagnostik IGRA dengan metode QFT – Plus pada Tuberkulosis anak. Penelitian deskriptif analitik dengan desain cross-sectional pada pasien terduga Tuberkulosis anak. Pada penelitian ini didapatkan 72 subyek yang memenuhi kriteria. Hasil pemeriksaan bakteri tahan asam dan Gen X-Pert pada mayoritas subjek adalah negatif. Hasil pemeriksaan Tuberculin Skin Test menunjukkan hasil positif hanya pada sebagian kecil subjek. Jumlah subjek yang memiliki hasil IGRA positif sejumlah 21 anak. Hasil perhitungan tabel 2x2 didapatkan sensitifitas 29,7%, spesifisitas 75%, dan akurasi 34,7%. Hasil perhitungan dengan kurva ROC didapatkan AUC untuk TB1 64,9% (IK 95%: 40,9 – 88,9%) dengan cut-off 0,095 untuk sensitivitas 70,3% dan spesifisitas 75%. AUC TB2 63,1% (IK 95%: 39,3 – 86,9%) dengan cut-off 0,115 untuk sensitivitas 62,5% dan spesifisitas 75%. AUC TB2 – TB1 46,9% (IK 95%: 27,8 – 66%) dengan cut-off 0,01 untuk sensitivitas 53,1% dan spesifisitas 50%. Pemeriksaan IGRA memiliki nilai diagnostik yang lemah pada Tuberkulosis anak sehingga sebaiknya tidak dijadikan pemeriksaan penunjang utama untuk mendiagnosis Tuberkulosis anak.

English Abstract

Indonesia is among the 30 countries with the highest tuberculosis burden because there is an array of cases including tuberculosis, TBC-HIV co-infection, and multi drug resistant tuberculosis. Tuberculosis in children did not get enough attention compared to the one in adult due to its difficulty in diagnosing. One of the newest methods to diagnose Tuberculosis is IGRA but there haven’t been many studies conducted to determine the diagnostic value of IGRA in children. The purpose of this study is to determine the diagnostic value of IGRA with QFT – Plus method in Tuberculosis in childhood. This is a descriptive analytic study with crosssectional design for children that are clinically diagnosed with Tuberculosis. This study obtained 72 subjects that meet the criteria. Majority of subjects has a negative acid-fast bacilli and Gen X-Pert test results. Only a small portion of subjects showed positive results in Tuberculin Skin Test. There are 21 children with positive IGRA results. It can be inferred from the cross-sectional table that IGRA has the sensitivity of 29.7%, specificity of 75%, and accuracy of 34.7%. Measurements using ROC curves yielded the AUC of 64.9% (95% CI: 40.9 – 88.9%) for TB1 with cut-off point 0.095 for sensitivity of 70.3% and specificity of 75%. TB2 AUC is 63.1% (95% CI: 39.3 – 86.9%) with cut-off point 0,115 for sensitivity of 62.5% and specificity of 75%. TB2 – TB1 AUC 46.9% (95% CI: 27.8 – 66%) with cut-off point 0.01 for sensitivity of 53.1% and specificity of 50%. IGRA has a low diagnostic value in diagnosing tuberculosis in childhood so IGRA should not be the main supporting examination to diagnose childhood tuberculosis.
