Abstract

Pendahuluan: Terapi utama uveitis anterior adalah steroid topikal, seperti prednisolon asetat 1%, tetapi pemberian jangka panjang memiliki banyak efek samping seperti katarak dan glaukoma. Imunomodulator topikal, seperti tacrolimus 0.03%, sebagai terapi alternatif untuk uveitis anterior masih belum banyak diteliti. Tacrolimus topikal terbukti memperbaiki kondisi pasien-pasien dengan penyakit alergi mata. Tujuan: Mengetahui perbedaan kadar IL-6 humor akuos pada tikus model endotoxin-induced uveitis (EIU) yang diinduksi LPS setelah pemberian prednisolon asetat 1% dan tacrolimus 0.03%. Metode: Penelitian eksperimental di laboratorium Biokimia Fakultas Kedokteran Universitas Brawijaya pada bulan Juni-Agustus 2018, dimana hewan coba (tikus) dibagi menjadi 4 kelompok yaitu: kelompok I adalah kelompok kontrol negatif, kelompok II adalah kelompok kontrol positif, kelompok III adalah kelompok yang diinduksi LPS dan diberi tetes prednisolon asetat 1%, kelompok IV adalah kelompok yang diinduksi LPS dan diberi salep tacrolimus 0.03%. Empat puluh delapan jam setelah induksi LPS, dilakukan pengambilan humor akuos sebanyak 10 μl kemudian dilakukan pengukuran kadar IL-6 dengan ELISA. Hasil: Nilai rata-rata kadar IL-6 humor akuos kelompok I, II, III, dan IV adalah 16.390 ± 2.093 ng/L, 31.374 ± 1.265 ng/L, 20.790 ± 0.754 ng/L, dan 22.755 ± 1.161 ng/L. Berdasarkan uji Tukey, tidak terdapat perbedaan kadar IL-6 yang signifikan antara kelompok prednisolon asetat 1% dan kelompok tacrolimus 0,03% (p>0.05, CI 95%) Kesimpulan: Tacrolimus 0.03% dapat menjadi terapi alternatif untuk uveitis anterior karena memberikan efek penurunan kadar IL-6 yang tidak berbeda signifikan dengan prednisolon asetat 1%.

English Abstract

Introduction: The main therapy for anterior uveitis is topical steroids, such as 1% prednisolone acetate, but long-term administration has many side effects such as cataracts and glaucoma. Topical immunomodulators, such as tacrolimus 0.03%, as an alternative therapy for anterior uveitis have not been widely studied. Topical tacrolimus has been shown to improve the condition of patients with allergic ocular diseases. Purpose: To determine differences in the levels of IL-6 aqueous humor in LPS-induced EIU model rats after administration of prednisolone acetate 1% and tacrolimus 0.03%. Method: Experimental research in the Biochemistry Laboratory of the Faculty of Medicine, Universitas Brawijaya in June-August 2018, where the experimental animals (rats) were divided into 4 groups: group I was the negative control group, group II was the positive control group, group III was the LPS-induced group and prednisolone acetate 1% eyedrops was given, group IV was the LPS-induced group and tacrolimus 0.03% ointment was given. Forty-eight hours after LPS induction, 10 μl of aqueous humor was taken and then IL-6 levels were measured by ELISA. Results: The mean values of IL-6 aqueous humor levels in groups I, II, III, and IV were 16.390 ± 2.093 ng/L, 31.374 ± 1.265 ng/L, 20.790 ± 0.754 ng/L, and 22.755 ± 1.161 ng/L. Based on the Tukey test, there was no significant difference in IL-6 levels between group of prednisolone acetate 1% and group of tacrolimus 0.03% (p>0.05, CI 95%). Conclusion: Tacrolimus 0.03% can be used as an alternative therapy for anterior uveitis because it gives the reduction effect of IL-6 level which is not significantly different with prednisolone acetate 1%.
