Abstract

Jenis ruang perawatan berpengaruh terhadap keberhasilan perawatan konservatif ibu dengan preeklampsia berat. Keberhasilan perawatan konservatif dipengaruhi oleh faktor kuantitas tidur. Tidur yang cukup merupakan terapi norfarmakologi yang berpengaruh dalam penurunan tekanan darah dan kadar norepineprin. Apabila Patologi ini diperparah dapat berpengaruh terhadap peningkatan tekanan darah yang berdampak pada kehamilan dan beresiko terhadap kondisi janin. salah satu faktor penilaian klinis luaran janin adalah apgar score bayi dan PaO2. Penelitian ini menggunakan desain cohort study pada 30 ibu hamil preeklamsia berat. Sampel dibagi menjadi dua kelompok yaitu 15 ibu di rawat di ruang perawatan kelas I dan 15 di ruang perawatan kelas III. Instrumen untuk mengumpulkan data lama tidur menggunakan format pengumpulan data, pemeriksaan kadar norepineprin menggunakan urin tampung 24 jam dan pengukuran PaO2 menggunakan sampel darah arteri umbilkalis serta penilaian apgar score dinilai menggunakan format penilain apgar score. Pemerikan PaO2 dan apgar score dinilai setelah terminasi kehamilan. Data penelitian ini dianalisis dan diintrepretasikan dengan menguji hipotesis menggunakan program computer SPSS windows release versi 23 Penelitian ini menghasilkan perbedaan yang bermakna terhadap lama tidur malam dan nilai apgar score neonatus Sedangkan ada perbedaan yang tidak bermakna yaitu kadar norepineprin dan PaO2 neonatus. Hasil pada penelitian ini menunjukan adanya hubungan yang bermakna antara lama tidur malam dengan kadar norepineprin ibu, lama tidur malam dengan apgar score neonatus, lama tidur malam dengan kadar PaO2, kadar norepineprin 24 jam sebelum terminasi kehamilan dengan PaO2 neonatus dan apgar score neontus serta ada hubungan bermakna antara apgar score neonatus dengan PaO2. Jenis ruang perawatan dapat mempengaruhi lama tidur malam pada ibu dengan preeklamsia berat. Gangguan lama tidur menyebabkan aktifitas saraf simpatik sehinggu memicu produksi kadar norepineprin berlebih dan berdampak terhadap peningkatan tekanan darah. Peningkatan tekanan darah yang tidak teratasi dapat meningkatkan risiko terhadap kondisi janin

English Abstract

This type of treatment room affects the success of conservative care of the woman with severe preeclampsia. The success of conservative care is influenced by the quantity factor of sleep. Adequate sleep is a norfarmacological therapy that has an effect on decreasing blood pressure and norepinephrine levels. If this pathology is aggravated it can affect the increase in blood pressure that affects pregnancy and is at risk of fetal condition. one of the factors of clinical assessment of fetal outcome is apgar score of infant and PaO2. This study used a cohort study design in 30 pregnant women with severe preeclampsia. The sample was divided into two groups: 15 mothers were treated in first and second class treatment rooms in class III treatment room. Instruments to collect sleep duration data using data collection format, norepinephrine assay using 24 hour urine collection and PaO2 measurement using umbilical artery blood sample and apgar score appraisal was assessed using apgar score appraisal format. PaO2 and apgar scores are assessed after termination of pregnancy. This research data is analyzed and interpreted by testing the hypothesis using SPSS windows release version 23 software This study resulted in significant differences in the duration of night sleep and the apgar score of neonates. While there was no significant difference between norepinephrine and neonate PaO2 levels. The results of this study showed a significant association between the length of night sleep and maternal norepinephrine, length of night sleep with apgar neonate score, length of nights sleep with PaO2 levels, norepineprin 24 hours before termination of pregnancy with neonate PaO2 and apgar score neontus and there is a relationship meaningful between apgar score of neonate and PaO2. This type of treatment room may affect the duration of night sleep in women with severe preeclampsia. Sleep disturbance causes sympathetic nerve activity over a week to trigger excessive production of norepinephrine and lead to increased blood pressure. Increased blood pressure that can not be resolved can increase the risk of fetal condition
