Abstract

Angiogenesis dipengaruhi oleh ekspresi protein VEGF-A dan HIF-1α di sekitar lingkungan tumor. Peristiwa tersebut didukung oleh sitokin terkait inflamasi seperti interleukin-6 (IL-6), interleukin-17 (IL-17), dan transforming growth factor β (TGF-β). Penelitian ini bertujuan untuk mengetahui efek ekstrak meniran terhadap ekspresi sitokin IL-6, IL-17, TGF-β pada limpa, serta ekspresi protein HIF-1α dan VEGF-A pada jaringan payudara mencit model kanker payudara. Mencit strain BALB/c diinjeksi senyawa DMBA pada daerah subkutan kelenjar payudara dengan dosis 0,015 mg/gram berat badan (BB). Induksi kanker dilakukan selama enam minggu. Desain penelitian menggunakan rancangan acak lengkap (RAL) meliputi kelompok normal, kanker (untreated), dan perlakuan ekstrak meniran dengan tiga dosis berbeda (treated groups). Analisis flow-cytometry digunakan untuk mengukur kadar sitokin IL-6, IL-17, dan TGF-β pada limpa. Uji immunohistokimia dilakukan untuk melihat ekspresi HIF-1α dan VEGF-A pada jaringan payudara. Uji statistik dilakukan dengan menggunakan one-way ANOVA dengan nilai signifikansi sebesar 5%. Uji beda nyata dilakukan dengan uji Tukey HSD menggunakan software SPSS 16. Hasil induksi DMBA menunjukkan beberapa karakter yang mengarah pada kanker payudara, yaitu adanya hiperplasia epitelium, infiltrasi sel imun, dan akumulasi jaringan ikat serabut di sekitar jaringan payudara. Induksi tersebut juga menyebabkan kenaikan signifikan pada ekspresi HIF-1α & VEGF-A, dan sitokin IL-6, TGF-β, & IL-17. Ekstrak Phyllanthus niruri (EPN) dosis sedang (C-D2) dan tinggi (C-D3) menyebabkan penurunan sitokin inflamasi serta ekspresi protein HIF-1α. Penelitian ini belum mampu membuktikan pengaruh EPN terhadap VEGF-A.

English Abstract

Angiogenesis are affected by the expression of VEGF-A and HIF-α protein in tumor microenvironment. This process was promoted by inflammatory-related cytokines such as interleukin-6 (IL-6), interleukin- 17 (IL-17), and transforming growth factor β (TGF-β). This research aims to understand the effect of P.niruri extract against inflammatory cytokines in spleen and the expression of VEGF-A and HIF-α in breast tissue. The BALB/c strain mice were injected with DMBA subcutaneously around mammary glands with the dose of 0,015 mg/gr mice weight for 6 weeks. Completely randomized design was used in this research, consisiting of normal, untreated, and 3 treated groups with different doses. The P.niruri extract were administered orally every day for 14 days. Flow-cytometry analysis were used to measure IL-6, IL-17, and TGF-β levels in speen. Immunohistochemistry were also utilized to observe the expression of HIF-α and VEGF in breast cancer histology. Statistical analysis were done using one-way ANOVA with a significant value of 5%. Tukey honest significant difference were also done using SPSS 16. The induction of DMBA showed several characteristics similar to that of breast cancer, which are hyperplasia epithelium, infiltration of immune cells, and accumulation of fibrous connective tissue. The induction also significantly increase HIF-1α & VEGF-A protein, and IL- 6, TGF-β, IL-17 cytokines. The extract of phyllanthus niruri in moderate and high dose were shown to decrease several inflammatory-related cytokines, and also the expression of HIF-1α. The extract did not cause any significant difference of VEGF-A expression.
