Pengaruh Penambahan Suplementasi Kurkumin Pada Pemberian Vitamin D3 Terhadap Kadar C3 Dan C4 Serta Aktivitas Penyakit Pasien Lupus Eritematosus Sistemik (Les) Dengan Hipovitamin D

Abstract

Latar Belakang: Vitamin D penting dalam regulasi sistem imun pasien LES. Kurkumin ligan VDR (vitamin D receptor) memiliki fungsi homolog dengan vitamin D. Tujuan penelitian menentukan pengaruh penambahan kurkumin pada pemberian vitamin D3 terhadap kadar C3, C4, dan vitamin D, serta aktivitas penyakit. Metode: Single-blind randomized controlled trial. Tiga puluh sembilan pasien LES hipovitamin D dibagi: 19 (kelompok suplementasi) diberi vitamin D 3x400 IU dan kurkumin 3x500mg; 20 (kelompok plasebo) vitamin D dan plasebo, selama tiga bulan. Pengukuran kadar C3, C4, vitamin D dengan ELISA, aktivitas penyakit dengan skor SLEDAI. Hasil dievaluasi dengan uji t berpasangan, Wilcoxon sign-rank dan uji korelasi Spearman’s rho. Hasil dan Pembahasan: Kadar C3 dan C4 setelah perlakuan tidak meningkat pada kedua kelompok, median C3 68,85 (IQR: 61,82-78,15) dan 8,83 (IQR: 7,69-33,71), p<0,05, sedangkan C4 0,0 (IQR:0,0-0,2) dan 0,1 (IQR: 0,0-0,2), p<0,05. Terjadi penurunan signifikan skor SLEDAI pada kelompok suplementasi median 12 (IQR: 8-22) dan 6 (IQR: 6-12), p<0,05, kelompok plasebo 14 (IQR: 9-18) dan 9 (IQR: 6-12), p<0,05. Kadar vitamin D meningkat dengan median 15,2 (IQR: 7,7-20,3) dan 24,4 (IQR: 13,0-29,0) vs 14,5 (IQR: 7,9-22,3 dan 26,0 (IQR: 24,3-28,0), berturut-turut, p<0,05. Penurunan skor SLEDAI tidak berkorelasi dengan peningkatan kadar vitamin D kedua kelompok (r=-0,415 vs r=0,123, p>0,05), yang mungkin disebabkan oleh kurang tepatnya pemberian dosis kurkumin atau penggunaan C3 dan C4 sebagai marker. Kesimpulan: Penambahan kurkumin tidak menunjukkan perbedaan signifikan pada kadar C3, C4, vitamin D, dan aktivitas penyakit. Perlu penelitian dengan penambahan kurkumin dosis bertingkat atau marker selain C3 dan C4.

English Abstract

Background: Vitamin D has important roles in SLE immune system. Curcumin as a VDR ligand has homolog function with vitamin D. This study aims to determine effect of curcumin addition in vitamin D3 administration on level of C3, C4, vitamin D, and disease activity. Method: single-blind randomized controlled trial. Thirty nine hypovitamin D SLE patients: 19 (supplementation group) who received vitamin D 3x400mg, with curcumin 3x500 mg; 20 (placebo group) were given vitamin D and placebo, for 3 months. Measurement of C3, C4, and vitamin D were using ELISA, disease activity using SLEDAI score. Results were evaluated with paired t-test, Wilcoxon sign-rank and Spearman’s rho correlation test. Result and Discussion: C3 and C4 didn’t increased in both group, C3 level median 68,85 (IQR: 61,82-78,15) and 8,83 (IQR: 7,69-33,71), p<0,05, C4 0,0 (IQR:0,0-0,2) and 0,1 (IQR: 0,0-0,2), p<0,05. Decreased SLEDAI in supplementation group were 12 (IQR: 8-22) and 6 (IQR: 6-12), p<0,05, while placebo group 14 (IQR: 9-18) and 9 (IQR: 6-12), p<0,05. Vitamin D level increased with median 15,2 (IQR: 7,7-20,3) and 24,4 (IQR: 13,0-29,0) vs 14,5 (IQR: 7,9-22,3 and 26,0 (IQR: 24,3-28,0)), respectively, p<0,05. Decreased SLEDAI scores have no correlation with increased vitamin D (r=0,415 vs r = -0,123, p>0,05), that might be caused by inaccurate curcumin dasage or C3 and C4 as marker. Conclusion: Addition of curcumin showed no significant difference toward level of C3, C4, vitamin D, and disease activity. Further research with gradual curcumin doses or using markers other than C3 or C4 should be considered.
