Abstract
PT Tirta Investama (AQUA) Pandaan memiliki beberapa permasalahan dalam proses produksi yang terjadi pada tahun 2017 dan 2018. Terdapat 4 permasalahan pada tahun 2017 yang disebabkan adanya kelalaian kerja yang dilakukan oleh karyawan, dan 1 permasalahan yang disebakan karena pekerjaan tidak dilakukan sesuai standar yang telah ditetapkan. Dan pada tahun 2018 terdapat 3 permasalahan yang disebabkan oleh kelalaian manusia. Setiap permasalahan yang ada di inputkan ke dalam sistem Behaviour Safety Analysis (BSA). Langkah pertama yang dilakukan dalam penelitian ini adalah melakukan wawancara terhadap SPV Performance and Method mengenai proses bisnis saat ini pada gedung Yudhistira, selanjutnya dari hasil wawancara dilakukan pemodelan proses bisnis menggunakan Business Process Model and Notation (BPMN). Selanjutnya setiap aktivitas yang terdapat pada proses bisnis dianalisis dengan menggunakan metode Failure Mode and Effect Analysis (FMEA) sehingga permasalahan pada setiap aktivitas dapat dianalisis secara mendalam. Setelah diperoleh hasil analisis menggunakan Failure Mode and Effect Analysis (FMEA), maka langkah selanjutnya yaitu mencari akar permasalahan dari setiap aktivitas dengan menggunakan Root Cause Analysis (RCA), setiap aktivitas di gambarkan menggunakan fishbone diagram, sehingga dapat diketahui kategori apa saja yang menyebabkan permasalahan bisa terjadi. Penggunaan RCA diharapkan dapat membantu perusahaan dalam melakukan perbaikan proses produksi sehingga dapat mengurangi permasalahan yang ada. Hasil dari penelitian ini terdapt 3 faktor yang menyebabkan terjadinya permasalahan pada proses produksi, yaitu kategori kelalaian manusia, kategori pengukuran, dan kategori metode. Namun faktor utama penyebab terjadi nya permasalahan saat proses produksi adalah kelalaian pekerja yang tidak mematuhi peraturan, terdapat 20 permasalahan dengan kategori kelalaian manusia. Karena kelalaian manusia merupakan faktor utama penyebab permasalahan, maka diberikan rekomendasi penambahan fitur pada sistem Behaviour Safety Analysis (BSA). Fitur yang ditambahkan adalah fitur Person Involved, fitur jumlah kelalaian, serta fitur kategori penyebab. Sehingga perusahaan mudah dalam melakukan evaluasi lebih lanjut pada setiap permasalahn yang terjadi serta mempermudah mengetahui karyawan yang sering melakukan kesalahan.
English Abstract
PT Tirta Investama (AQUA) Pandaan has several problems in the production process that occurred in 2017 and 2018. There are 4 problems in 2017 due to the negligence of work carried out by employees, and 1 problem caused because the work was not carried out according to the standards set. And in 2018 there were 3 problems caused by human negligence. Every problem is input into the Behavior Safety Analysis (BSA) system. The first step taken in this study was to conduct interviews with SPV Performance and Method regarding current business processes in the Yudhistira building, then from the results of interviews business process modeling was conducted using Business Process Model and Notation (BPMN). Furthermore, each activity contained in the business process is analyzed using the Failure Mode and Effect Analysis (FMEA) method so that the problems in each activity can be analyzed in depth. After obtaining the results of analysis using Failure Mode and Effect Analysis (FMEA), the next step is to find the root cause of each activity using Root Cause Analysis (RCA), each activity is described using a fishbone diagram, so that any categories can cause problems. could occur. The use of RCA is expected to help companies in improving the production process so as to reduce existing problems. The results of this study found three factors that caused problems in the production process, namely the category of human negligence, measurement category, and method category. But the main factor causing the problem when the production process is negligence of workers who do not comply with the regulations, there are 20 problems with the category of human negligence. Because human negligence is the main factor causing problems, a recommendation is to add features to the Behavior Safety Analysis (BSA) system. The added feature is the Person Involved feature, the number of negligence features, and the cause category features. So that the company is easy to carry out further evaluation on every problem that occurs and makes it easier to find out employees who often make mistakes.
