Abstract
Non Alcoholic Fatty Liver Disease (NAFLD) berkaitan erat dengan sindrom metabolik yang ditandai dengan adanya akumulasi lemak di hepar dan kemudian memicu respon inflamasi, stress oksidatif dan fibrosis lanjut. Hingga saat ini beluma ada terapi yang efetif untuk NAFLD. Terapi hanya sebatas menangani dislipidemia dengan menggunakan obat gologan statin. Penelitian ini bertujuan untuk mengetahui pengaruh dari ekstrak mikroalga Dunaliella sp. yang mengandung senyawa antioksidan terhadap kadar sitokin proinflamasi yang terlibat dalam patogenesis NAFLD yaitu Tumor Necrosis Factor Alfa (TNF-α) di hepar. Penelitian ini menggunakan randomized post test only controlled group design untuk mengetahui pengaruh ekstrak Dunaliella sp. terhadap kadar TNF-α hepar. Tikus wistar sebanyak 30 ekor dibagi ke dalam 6 kelompok yaitu; kelompok kontrol negatif, kelompok kontrol positif yang diinduksi sindrom metabolik dengan diet tinggi lemak dan minum fruktosa selama 8 minggu (MetS); kelompok MetS + ekstrak Dunaliella sp. 500 mg/kgBB; MetS + ekstrak Dunaliella sp. 1000 mg/kgBB; MetS + ekstrak 1500 mg/kgBB yang diberikan pada minggu ke-5; Mets + simvastatin 4mg/kgBB. Organ hepar diambil kemudian kadar TNF-α hepar diukur dengan metode ELISA. Hasil penelitian menunjukkan bahwa induksi sindrom metabolik meningkatkan inflamasi di hepar dengan tingginya kadar TNF-α secara signifikan (p<0.05) dibandingkan kontrol negatif dan ekstrak Dunaliella sp. dapat menurunkan kadar TNF-α hepar secara signifikan dibandingkan dengan kontrol positif (p<0.05), khususnya pada dosis 500 mg/kgBB. Kesimpulan dari penelitian ini adalah ekstrak Dunaliella sp. menurunkan kadar TNF-α hepar tikus wistar sindrom metabolik sebagai agen pencetus inflamasi.
English Abstract
Non Alcoholic Fatty Liver Disease (NAFLD) is closely related to metabolic syndrome and characterized by hepatic fat accumulation which could sequentially trigger inflammatory response, oxidative stress, and advanced fibrosis. Until now there has been no effective therapy for NAFLD. The therapy is limited to treat dyslipidemia only by using statins. This study aims to determine the effect of Dunaliella sp. extract which contain antioxidant compounds to the levels of proinflammatory cytokines which involved in the pathogenesis of NAFLD, namely Tumor Necrosis Factor Alpha (TNF-α) in hepatic tissue. This study is randomized post test only controlled group design as 30 wistar rats were divided into 6 groups, namely; negative control group, positive control group induced by metabolic syndrome with combination of high-fat diet and fructose drinking water for 8 weeks (MetS); MetS + Dunaliella sp. 500 mg / kgBB; MetS + extract of Dunaliella sp. 1000 mg / kgBB; MetS + extract 1500 mg / kgBB respectively started from 5th week; MetS + simvastatin 4mg / kgBB. Hepatic tissue was obtained and TNF-α levels were measured by using ELISA method. The result shows that metabolic syndrome rats in positive control group showed hepatic inflammation as indicated by significantly (p<0.05) increased hepatic levels of TNF-α compared to negative controls. At the same time, interestingly, treatment with Dunaliella sp extract at doses 500 mg/kgBB can significantly (p<0.05) decrease hepatic TNF-α levels compared with positive control group. In conclusion, Dunaliella sp. treatment decrease levels of hepatic TNF-α of wistar rats metabolic syndrome thus reduce the hepatic inflammatory response
