Abstract

Tuberkulosis (TB) merupakan penyakit yang disebabkan oleh Mycobacterium tuberculosis dan menyerang paru. Salah satu penetapan diagnosis penyakit TB paru adalah dengan pemeriksaan radiologik. Berdasarkan pemeriksaan radiologik, TB paru dibedakan menjadi lesi luas dan lesi minimal. Pada pemeriksaan darah didapatkan monositosis dan peningkatan laju endap darah (LED). Tujuan penelitian ini untuk Mengidentifikasi perbedaan jumlah monosit dan LED pada penderita tuberkulosis paru lesi luas, lesi minimal dan pasien sehat di RSUD dr. Saiful Anwar Malang. Jenis penelitian observasional analitik dengan pendekatan croos sectional. Jumlah sampel penelitian sebanyak 40 kelompok kasus yaitu 10 TB paru lesi minimal, 10 TB paru lesi luas dan 20 kelompok kontrol. Analisis data meliputi analisis deskriptif dan uji hipotesis, menggunakan uji Chi-squere dan Mann Whitney. Untuk melihat hubungan menggunakan uji korelasi. Hasil penelitian, uji Kruskal Wallis chi-square ditemukan minimal ada satu pasang kelompok pasien yang memberikan pengaruh yang berbeda signifikan terhadap jumlah monosit (P=0,002), dan terhadap jumlah LED (p = 0,000). Uji Mann Whitney jumlah monosit ditemukan tidak ada perbedaan antara kelompok TB Paru lesi luas dengan kelompok TB paru lesi minimal (p = 0,436), tetapi kelompok TB paru lesi luas dan minimal berbeda signifikan dengan kelompok pasien sehat (p= 0,001 dan p= 0,017). Uji Pengujian Mann Whitney jumlah LED ditemukan ada perbedaan signifikan antara kelompok tuberkulosis luas dengan kelompok tuberkulosis minimal (p = 0,0043), dan didapatkan juga perbedaan signifikan antara kelompok TB paru lesi luas dan TB paru lesi minimal dengan kelompok sehat (p = 0,000 dan p = 0,001). Uji korelasi adanya hubungan yang cukup kuat kelompok pasien dengan jumlah monosit dan ada hubungan yang kuat kelompok pasien TB dengan LED.

English Abstract

Tuberculosis (TB) is a disease caused by Mycobacterium tuberculosis and attacks the lungs. One of the determinations of pulmonary TB disease diagnosis is by radiological examination. Based on radiological examination, pulmonary TB is divided into extensive lesions and minimal lesions. In the blood test, increasing number of monocytes are obtained and increase in blood sedimentation rate (LED). The purpose of this study was to identify differences in the number of monocytes and LEDs in patients with pulmonary tuberculosis extensive lesions, minimal lesions and healthy patients in hospitals dr. Saiful Anwar Malang. Type of observational analytic research with cross sectional approach. The number of samples were 40 divided 10 of minimal lung lesions, 10 of pulmonary tuberculosis lesions and 20 of control groups. Data analysis included descriptive analysis and hypothesis testing, using Chi-square and Mann Whitney test. To see the relationship using correlation test. Result of research, Kruskal Wallis chi-square test found that there is at least one pair of patient group which gives significant different effect to monocyte count (p = 0.002), and to LEDs number (p= 0.000). Mann Whitney test of monocyte count was found to be no difference between groups of TB Lung extensive lesion with minimal lung TB group lesions (p = 0.436), but the group of pulmonary tuberculosis extensive lesions were and minimal lesion significantly different with healthy patient group (p= 0.001 and p = 0.017). Testing of Mann Whitney Test for the number of LEDs found no significant differences between tuberculosis extensive lesion group and tuberculosis minimal lesion group (p = 0.0043), and also found significant differences between tuberculosis extensive lesion group and minimal lesion group with healthy group (p = 0.000 and p = 0.001). The correlation test shown there is strong association between tuberculosis patients with the number of monocytes and there is a strong association between tuberculosis patients with LEDs.
