Abstract
Metotreksat merupakan pengobatan lini pertama yang diberikan pada pasien dengan diagnosis artritis reumatoid dengan alasan pemberian dapat digunakan sebagai monoterapi maupun kombinasi hingga mampu mencapai kondisi remisi.Tujuan dari penelitian ini adalah untuk mengetahui hubungan dosis dan lama penggunaan metotreksat terhadap aktivitas artritis reumatoid dengan pemantauan aktivitas penyakit melalui Disease Activity Score 28 (DAS28). Penelitian menggunakan metode potong lintang dengan analisis deskriptif kuantitatif dengan mengambil tempat di Poli Rawat Jalan bagian Reumatologi Rumah Sakit Saiful Anwar (RSSA) Kota Malang pada bulan Maret–Mei 2018. Pengujian statistik korelatif menggunakan statistik parametrik Pearson untuk lama penggunaan dan dosis rata-rata dan non parametrik Rank-Spearman untuk dosis kumulatif dan dosis maksimal Terdapat hubungan negatif antara dosis kumulatif (r = -0,091 ; p = 0,400) dan lama penggunaan (r = -0,075 ; p = 0,489) serta hubungan positif antara dosis rata–rata (r = 0,043 ; p = 0,692) dan dosis maksimal (r = 0,074 ; p = 0,492) terhadap aktivitas hasil skoring DAS28 dengan tingkat hubungan sangat lemah. Adapun pengujian statistik non hipotesis berupa uji beda Kruskal Wallis dilakukan dengan hasil bahwa tidak terdapat perbedaan pada pemberian dosis yang berbeda pada dosis kumulatif (p = 0,642), dosis rata–rata (p = 0,912), dan dosis maksimal (p = 0,584) serta lama penggunaan (p = 0,101) terhadap skoring DAS28. Dapat disimpulkan bahwa terdapat hubungan negatif dosis kumulatif dan lama penggunaan serta hubungan positif dosis ratarata dan dosis maksimal terhadap nilai DAS28.
English Abstract
Methotrexate used to as first line choice therapy given to patient who diagnosed with rheumatoid arthritis with underlying reason can be used to as mono therapy or combination with other drugs and continuously using until remission condition reached by patient. The objective for this study is for acknowledging the relationship between methotrexate dose and length of therapy toward to rheumatoid arthritis activity monitored with Disease Activity Score 28 (DAS28). Cross sectional with quantitative analysis description was used in this research method and conducted at Rheumatology Polyclinic of Saiful Anwar Hospital (RSSA) Malang in the given time March until May 2018. Statistic correlation analysis using both parametric Pearson and non parametric Rank-Spearman. The result was, there are negative correlation between cumulative dose (r = - 0,091 ; p = 0,400), and length of therapy (r = -0,075 ; p = 0,489) and also positive correlation for average dose (r = 0,043 ; p = 0,692) and maximum dose (r = 0,074 ; p = 0,492) toward to arthritis rheumatoid activity monitored with DAS28 scoring. Result from statistical analysis Kruskal Wallis showed that there wasn’t any significantly different in a cumulative dose, average dose, maximal dose, and length of therapy. Research result showing remission condition only achieved by 16 out of 88 patients with the underlying reason non adequate starting dose and other prognosis factor. Conclusion from this research is there are negative correlation for cumulative dose, length of therapy. Also negative correlation for average and maximum dose toward to rheumatoid arthritis activity.
