Abstract

Lupus adalah penyakit autoimun yang merusak multisistem secara kronis, sehingga sering menimbulkan komplikasi pada ginjal disebut Lupus Nefritis (LN). Lupus nefritis terjadi pada 60% pasien lupus dan tetap menjadi penyebab utama morbiditas pada pasien lupus di seluruh dunia 5 tahun terakhir, tergantung respon terhadap pengobatan awal. Pengobatan LN paling efektif saat ini adalah Mikofenolat Mofetil (MMF) dan Inhibitor Kalsineurin, tunggal maupun kombinasi. Semakin tinggi derajat penyakit, semakin besar risiko terjadinya kegagalan terapi, sehingga dibutuhkan terapi agen biologis. Tetapi, agen biologis di Indonesia masih sulit diterapkan karena harga yang mahal. Terapi biosimilar dengan ekstrak etanol daun cocor bebek (Bryophyllum pinnatum) diduga akan menyerupai target terapi agen biologis, yaitu menurunkan aktivitas autoantibodi pada ginjal. Penelitian memiliki tujuan untuk dapat membuktikan efek pemberian ekstrak etanol daun B. pinnatum terhadap perbaikan jaringan ginjal mencit Balb/c model lupus induksi pristane. Desain penelitian eksperimental di laboratorium menggunakan rancangan post test only controlled group design secara in vivo. Sampel dibagi menjadi 5 kelompok, yaitu kontrol negatif, kontrol positif, perlakuan 1 (10,5 mg/kgBB/hari), perlakuan 2 (21 mg/kgBB/hari), perlakuan 3 (42 mg/kgBB/hari). Pada awal penelitian, kelompok kontrol positif dan kelompok perlakuan diinjeksi pristane sesuai ketentuan. Pada bulan ke-4 setelah injeksi pristane, dilakukan pemberian ekstrak etanol daun B. pinnatum secara oral selama 3 bulan perlakuan. Selanjutnya dilakukan pembedahan untuk mengambil organ ginjal. Kemudian dibuat slide dengan pengecatan hematoxylin and eosin (H&E) untuk diamati secara morfometrik dengan mengukur diameter glomerulus. Hasil penelitian menunjukkan adanya perbedaan yang bermakna antara diameter glomerulus pada mencit kontrol negatif, kontrol positif, dan kelompok perlakuan (ANOVA p-0,000).

English Abstract

Lupus is an autoimmune disease that damages multisystems chronically, so it often causes kidney complications called Lupus Nephritis (LN). Lupus nephritis occurs in 60% of lupus patients and remains a major cause of morbidity in lupus patients worldwide over the past 5 years, depending on the response to initial treatment. The most effective LN treatment currently is Mycophenolate Mofetil (MMF) and calcineurin inhibitors, single or combination. The higher the degree of disease, the greater the risk of therapy, so biological agent therapy is needed. However, biological agents in Indonesia are still difficult to implement because of the high prices. Biosimilar therapy with Bryophyllum pinnatum leaves ethanol extract is thought to resemble the target of biological agent therapy, which is reducing the activity of autoantibodies in the kidneys. The study aims to prove the effect of B. pinnatum leaves ethanol extract on repair of kidney tissue in pristane-induced lupus Balb/c mice model. The experimental study design in the laboratory used a post-test only controlled group design in vivo. Samples were divided into 5 groups: negative control, positive control, treatment 1 (10.5 mg/kgBW/day), treatment 2 (21 mg/kgBW/day), treatment 3 (42 mg/kgBW/day). At the beginning of the study, the positive control group and the treatment groups were injected pristane in accordance with the provisions. In the 4th month after pristane injection, then the B. pinnatum leaves ethanol extract was orally administered for 3 months of treatment. After that, surgery is performed to take kidney organs. Then slides were made with hematoxylin and eosin (H&E) staining to be observed morphometrically by measuring the glomerular diameter. The results shows a significant difference between glomerular diameter among negative control, positive control, and treatment group (ANOVA p-0,000).
