Abstract

Penderita diabetes berpotensi mengalami komplikasi kaki diabetik akibat neuropati sensori sebesar 40%-70%. Terapi pecegahan keparahan komplikasi kaki diabetik yaitu dengan intervensi NMT. Intervensi NMT memberikan stimulasi pada kulit dan jaringan dibawahnya yang menyebabkan pembuluh darah dilatasi sehingga dapat memperbaiki status sensori. Penelitian ini bertujuan untuk mengetahui pengaruh penerapan NMT terhadap status sensori kaki diabetik pada pasien DM tipe 2 di wilayah kerja Puskesmas Kota Malang. Penelitian ini menggunakan metode true eksperiment dengan desain randomized control group pretest-posttest. Jumlah responden penelitian ini, yaitu 50 responden dengan dibagi menjadi 2 kelompok, yaitu 25 responden kelompok intervensi dan 25 responden kelompok kontrol. Kelompok intervensi diberikan intervensi NMT selama 1 minggu dengan 3 kali pergantian plester, sedangkan kelompok kontrol tidak diberikan intervensi NMT. Berdasarkan hasil penelitian diperoleh status sensori kaki kanan kelompok kontrol rata-rata menurun 2,000 dan kaki kiri menurun 2,120. Penurunan status sensori terjadi karena berbagai faktor, misalnya usia, lama DM, kolesterol total dalam darah, dankadar glukosa darah. Status sensori kaki kanan kelompok intervensi rata-rata meningkat 3,960 dan kaki kiri meningkat 4,160, peningkatan status sensori disebabkan oleh penerapan NMT selama 1 minggu. Hasil uji Independent T-Test menunjukkan bahwa terdapat perubahan status sensori yang sama antara kaki kanan dan kaki kiri kelompok intervensi dan kelompok kontrol, yaitu dengan nilai p-value 0,000 dengan α 0,05 (p-value < 0,05). Berdasarkan hasil tersebut dapat disimpulkan bahwa terdapat pengaruh penerapan NMT terhadap status sensori kaki diabetik pada pasien DM tipe 2 di wilayah kerja Puskesmas Kota Malang.

English Abstract

People with diabetes melitus potentially have diabetic foot complications from sensory neuropathy by 40%-70%. Prevention therapy severity of diabetic foot complications by NMT intervention. NMT intervention provided stimulation of the skin and underlying tissues that cause dilated blood vessels to improve the sensory status. This study aims to determine the effect of NMT application on diabetic foot sensory status in type 2 DM patients in the working area of Puskesmas Malang City. This research uses true experiment method with randomized control group pretest-posttest design. The respondents of this study, which 50 respondents divided into 2 groups, there is 25 respondents of the intervention group and 25 respondents control group. The intervention group was given NMT intervention for 1 week with three time change of plaster, while control group was not given NMT intervention. Based on the research results obtained sensory status right foot control group decreased 2,000 and left leg decreased 2,120. Decreased sensory status occurs due to various factors, such as age, long DM, total cholesterol in the blood, and blood glucose levels. The right sensory status of the right arm of the intervention group increased by an average of 3.960 and the left leg increased 4,160, the increase in sensory status caused by the application of NMT for 1 week. The results of the Independent T-Test showed that there was a change in the same sensory status between the right and left foot of the intervention group and the control group, with p-value 0,000 with α 0.05 (p-value <0.05). Based on these results it can be concluded that there is influence of the application of NMT to diabetic foot sensory status in patients with type 2 diabetes in the work area of Puskesmas Malang.
