Abstract

Nodul tiroid adalah temuan klinis yang sering ditemui di masyarakat. Meskipun kasus keganasan dari sel epitel tiroid relatif jarang, tetapi karsinoma tiroid merupakan keganasan endokrin yang paling sering terjadi dari seluruh keganasan endokrin. Biopsi Aspirasi Jarum Halus adalah pemeriksaan awal yang dapat digunakan untuk mendiagnosa tumor jinak dan ganas tiroid. Penelitian ini bertujuan untuk mengetahui nilai diagnostik FNAB untuk membedakan tumor jinak dan tumor ganas tiroid dengan baku emas histopatologi serta menentukan titik potong skor gambaran FNAB pada tumor jinak dan tumor ganas tiroid dengan baku emas histopatologi. Penelitian ini bersifat observasional analitik dengan pendekatan cross sectional. Uji diagnostik menggunakan tabel 2x2 dan kurva ROC (Reciever Operating Characteristic) sehingga didapatkan nilai sensitivitas, spesifitas, akurasi dan AUC (Area Under Curve). Sampel terdiri dari 35 sampel. Sampel dibaca ulang dan diberi skor sesuai kriteria gambaran sitologi, kemudian Hasil total skor pada pembacaan FNAB akan dibandingkan dengan hasil diagnosis hasil histopatologi menggunakan aplikasi SPSS(Statistical Package for the Social Science). FNAB tanpa sistem skor memiliki nilai sensitivitas sebesar 77,78% , spesifitas 88,46%, dan akurasi sebesar 85,71% dengan nilai duga positif sebesar 70% dan nilai duga negatif sebesar 92%.Skoring FNAB dengan cut-off ≥5 memiliki nilai sensitivitas 66,67%, nilai spesifisitas 100%, nilai duga positif 100%, nilai duga negatif 89,65% dan akurasi 91,42%. Fine Needle Aspiration Biopsy (FNAB) memilik nilai diagnostik yang tinggi dengan nilai AUC sebesar 90,4% untuk membedakan tumor jinak dan tumor ganas tiroid dibandingkan dengan baku emasnya, yaitu histopatologi. Pemeriksaan FNAB dengan sistem skoring mempunyai nilai diagnostik yang lebih baik dibandingkan FNAB tanpa sistem skoring tetapi belum bisa menggantikan histopatologi sebagai baku emas

English Abstract

Thyroid Nodules is a clinical finding that is commonly found in society. Even though cases of thyroid’s epithelial cells’ malignancy are thought as a rarity, thyroid carcinoma is an endrocine malignancy, of which, is considered relatively often in term of occurance among other endocrine malignancies. The Fine Needle Aspiration Biopsy is an early examination that can be used to diagnose benign and malignant thyroid tumors. This research was conducted to examine the diagnostic value of FNAB which later would distinguish between malignant and benign thyroid tumors, and determine the cut off point score of FNAB on benign and malignant thyroid tumors, both with the gold standard of histopathology. This study was observational analytics in nature accompanied with cross sectional approach. The diagnostic test used 2x2 table and ROC (Receiver Operating Characteristic) curve to obtained sensitivity score, specificity, accuracy, and AUC (Area Under Curve). There were 35 samples used in this study. Each sample was reread and given scores according to the cytologic criterias, the total score results of FNAB readings would later be compared with diagnostic results of histopathology using SPSS. FNAB without scoring system had sensitivity value of 77.78%, specificity of 88.46% and 85.71% of accuracy with positive predictive value of 70% and negative predictive value of 92%. FNAB with cut off of ≥5 had 66.67% sensitivity value, specificity and positive predictive value of 100%, negative predictive value of 89.65%, and accuracy of 91.42%. Fine Needle Aspiration Biopsy (FNAB) has high diagnostic value with the AUC value of 90.4% to distinguish between benign and malignant thyroid tumors compared to its gold standard, histopathology. FNAB examination with scoring system has a better diagnostic value in contrast of FNAB without scoring sytem, yet it is not able to replace histopathology as the gold standard.
