Abstract

Demam tifoid merupakan penyakit yang disebabkan oleh infeksi Salmonella typhi. Terapi yang digunakan pada pengobatan demam tifoid anak adalah kloramfenikol, sefiksim, seftriakson, tiamfenikol, dan sefotaksim. Kloramfenikol merupakan terapi standar pada pengobatan demam tifoid anak, namun memiliki kekurangan yakni dapat menimbulkan kekambuhan dan efek samping yaitu anemia aplastik. Sefotaksim dinilai dapat direkomendasikan sebagai obat alternatif pada kasus resistensi kloramfenikol. Penelitian ini bertujuan untuk mengetahui efektivitas dan efisiensi biaya dari penggunaan kloramfenikol dan sefotaksim pada pasien demam tifoid anak di RSU Karsa Husada Batu dengan menggunakan analisis cost-effectiveness. Adapun metode yang digunakan yaitu observasional analitik dengan sudut pandang penyedia pelayanan kesehatan secara retrospektif menggunakan data rekam medis pasien. Terdapat 27 pasien yang masuk dalam penelitian ini, yaitu 22 pasien menggunakan kloramfenikol dan 5 pasien menggunakan sefotaksim. Berdasarkan lama rawat inap, nilai ACER kloramfenikol sebesar Rp 449.420,60 per hari sedangkan sefotaksim sebesar Rp 386.821,05 per hari. Berdasarkan lama hilang gejala demam, nilai ACER kloramfenikol sebesar Rp 474.171,30 per hari sedangkan sefotaksim sebesar Rp 432.329,41 per hari. Berdasarkan nilai ICER, penggunaan sefotaksim membutuhkan tambahan biaya sebesar Rp 4.049,16 untuk menghasilkan pengurangan 1 hari lama rawat inap dan menurunkan biaya sebesar Rp 9.196,02 untuk menghasilkan pengurangan 1 hari lama hilangnya gejala demam. Dari hasil penelitian ini dapat disimpulkan bahwa sefotaksim lebih cost-effective dibandingkan dengan kloramfenikol dalam pengobatan demam tifoid anak.

English Abstract

Typhoid fever is a disease caused by Salmonella typhi infection. Therapy used in the treatment of pediatric typhoid fever are chloramphenicol, cefixime, ceftriaxone, tiamphenicol, and cefotaxime. Chloramphenicol is standard theraphy in the medication of pediatric typhoid fever, but it has some disadvantages that could cause recurrence and hige risk side effects anemia aplastic. Cefotaxime is considered to be recommended as an alternative therapy in cases of chloramphenicol resistance. This research aimed to determine the effectiveness and cost-effective from usage of chloramphenicol and cefotaxime in pediatric typhoid fever patient at RSU Karsa Husada Batu by using cost-effectiveness analysis. The method applied was observational analytics with the viewpoint of healthcare providers retrospectively using patients medical record data. There were 27 patients included in this study, 22 patients were given chloramphenicol and 5 patients were given cefotaxime. Based on the length of stay, the ACER value of chloramphenicol was Rp 449.420,60 per day, while the value of cefotaxime was Rp 386.821,05 per day. Based on long-lost fever’s symptoms, the ACER value of chloramphenicol was Rp 474.171,30 per day, while the value of cefotaxime was Rp 432.329,41 per day. Based on ICER value, the use of cefotaxime requires an additional cost by Rp 4.049,16 to reduce 1 day of hospitalization, and could reducing cost by Rp 9.196,02 to reduce 1 day of long-lost fever’s symptoms. From the result of this research, can be concluced that cefotaxime is more cost-effective than chloramphenicol in medication for pediatric typhoid fever.
