Abstract
Hiperglikemia kronis pada pasien diabetes melitus menyebabkan komplikasi berupa neuropati diabetik yang disertai ulkus kaki diabetik dan nyeri. Gejala yang banyak dijumpai pada pasien DM dengan nyeri adalah alodinia dan hiperalgesia. Rasa nyeri ini menjadi salah satu pencetus peningkatan hormon glukokortikoid yang dapat menghambat proses penyembuhan luka. Perawat berperan penting dalam penatalaksanaan nyeri, salah satu intervensi nyeri non farmakologi yang dapat diberikan adalah aplikasi neuromuscular taping (NMT). Mekanisme kerja NMT menstimulasi mekanoreseptor di kulit yang menyebabkan reaksi gate control sehingga nyeri berkurang. Penelitian ini bertujuan untuk mengetahui pengaruh aplikasi NMT terhadap nyeri neuropati diabetik. Desain penelitian ini adalah quasi eksperiment. Metode pengambilan sampel dengan consequtive sampling menggunakan pre tes-post tes control group desain. Jumlah sampel 38 orang, dibagi menjadi dua kelompok yaitu, kelompok kontrol dan intervensi. Kedua kelompok diberikan perawatan luka dengan teknik dan dressing yang sama, tetapi pada kelompok intervensi diberi tambahan NMT selama 7 hari dengan 1 kali penggantian plester. Berdasarkan hasil analisis penelitian diperoleh skor nyeri kelompok kontrol meningkat dengan P-value 0,317 (α = 0,005). Skor nyeri kelompok intervensi menurun dengan P-value 0,000 (α = 0,005). Hasil uji Mann-whitney terhadap skor nyeri kelompok kontrol dan intervensi menunjukkan perubahan yang signifikan dengan P-value 0,000 (α=0,05). Kesimpulannya adalah ada pengaruh aplikasi NMT terhadap nyeri neuropati diabetik pada pasien ulkus kaki diabetik. Penelitian ini merekomendasikan perawat untuk melakukan aplikasi NMT dalam penatalaksanaan nyeri neuropati diabetik pada ulkus kaki diabetik.
English Abstract
Chronic hyperglycemia in patients with diabetes mellitus causes complications in the form of diabetic neuropathy accompanied by diabetic foot ulcers and pain. Symptoms that are often found in DM patients with pain are allodynia and hyperalgesia. This pain becomes one of the triggers for an increase in glucocorticoid hormones which can inhibit the wound healing process. Nurses play an important role in the management of pain, one of the non-pharmacological pain interventions that can be given is the application of neuromuscular taping (NMT). The mechanism of action of NMT stimulates the mechanoreceptors on the skin which causes a gate control reaction so that pain decreases. This study aimed to determine the effect of NMT application on diabetic neuropathy pain. The design of this study is quasi experiment. Sampling method with consecutive sampling using pre test post control group design. The number of samples was 38 people, divided into two groups, namely, the control and intervention groups. Both groups were given wound care with the same technique and dressing, but in the intervention group were given additional NMT for 7 days with 1 replacement of plaster. Based on the results of the research analysis obtained the control group pain score increased with P-value 0.317 (α = 0.005). The intervention group pain score decreased with P-value 0,000 (α = 0.005). The Mann-Whitney test results on the pain score of the control group and the intervention showed significant changes with P-value 0,000 (α = 0.05). The conclusion is that there is an effect of NMT applications on diabetic neuropathic pain in diabetic foot ulcer patients. This study recommends that nurses conduct NMT applications in the management of diabetic neuropathy pain in diabetic foot ulcers.
