Abstract

Penyakit jantung koroner (PJK) merupakan keadaan terjadinya penimbunan plak aterosklerosis pada pembuluh darah koroner. Simvastatin digunakan karena efek pleiotropik untuk menstabilkan plak aterosklerosis. Beberapa penelitian menyebutkan statin dapat mempengaruhi kadar glukosa darah pada pasien PJK sebagai tanda terjadinya kejadian baru diabetes (New Onset Diabetes). Penelitian ini dilakukan untuk membuktikan adanya perbedaan kadar glukosa darah pada pasien PJK-DM dan pasien PJK-nonDM yang mendapatkan simvastatin minimal 1 tahun di Poli Spesialis Jantung Rumah Sakit UNISMA Malang. Teknik pengambilan sampel adalah simple random sampling yang dipilih secara acak dari data pasien yang telah memenuhi kriteria inklusi. Penelitian melibatkan 44 pasien yang dibagi kedalam 2 kelompok yaitu PJK-DM dan PJK-nonDM. Pemeriksaan kadar glukosa darah (GDP dan GD2JPP) dilakukan tiap bulan sebanyak 3 kali. Data dianalisis secara statistik menggunakan aplikasi SPSS untuk uji normalitas dan uji hipotesis komparatif Mann Whitney, Friedman, dan Repeated Anova. Hasil dari uji hipotesis didapatkan bahwa adanya perbedaan yang bermakna pada kadar glukosa darah (GDP dan GD2JPP) antara pasien PJK-DM dengan pasien PJK-nonDM. Didapatkan juga bahwa perbedaan kadar GDP dan GD2JPP pada pasien PJK-DM setiap bulannya lebih tidak terkontrol dibandingkan dengan PJK-nonDM. Meskipun demikian kadar glukosa darah pada pasien PJK-nonDM belum mempresentasikan adanya diabetes kejadian baru (New Onset Diabetes). Faktor lain yang dapat mempengaruhi kadar glukosa darah antara lain usia, genetik, sifat statin, dan penggunaan obat antidiabetes. Kekurangan dalam penelitian seperti waktu penelitian, kepatuhan pasien, makanan, dan lama waktu puasa juga dapat mempengaruhi hasil pemeriksaan kadar glukosa darah.

English Abstract

Coronary heart disease (CHD) is a condition of accumulation of atherosclerosis plaque in coronary arteries. Simvastatin is used as a pleiotropic effect to stabilize atherosclerotic plaque. Some research said statins can affect blood glucose levels in CHD patients as the new occurrence of diabetes (New Onset Diabetes). This research was conducted to prove the differences in blood glucose between levels in patients with CHD-DM and CHD non-DM patients who received simvastatin for at least 1 year at the Poly Cardiology Specialist at Hospital UNISMA Malang. The sampling technique is a simple random sampling selected randomly from patient data that have met the inclusion criteria. The study involved 44 patients divided into 2 groups namely CHD-DM and CHD non-DM. Blood glucose (GDP and GD2JPP) tests were performed for 3 consecutive months. Data were analyzed statistically using SPSS application to test for normality and comparative hypothesis testing Mann Whitney, Friedman, and Repeated Anova. The results of hypothesis showed that there were significant differences in blood glucose levels (GDP and GD2JPP) between CHD-DM patients and CHD non-DMD patients. It was also found that differences in GDP and GD2JPP levels in CHD-DM patients each month were more uncontrolled compared to CHD non-DM. Even though, blood glucose levels in CHD non-DM patients have not presented a new incidence of diabetes (New Onset Diabetes). Other factors that can affect blood glucose levels include age, genetics, statin properties, and use of antidiabetic drugs. Shortcomings in studies such as the timing of the study, patient compliance, food, and duration of fasting may also affect the results of blood glucose levels.
