Abstract
Perusahaan CV. Aneka Grafika merupakan salah satu perusahaan yang bergerak dibidang percetakan khususnya pada percetakan kemasan karton. Permasalahan yang sering terjadi pada perusahaan yaitu target produksi sebagian besar tidak tercapai yang dapat menghambat kegiatan produksi. Hal tersebut terjadi karena terdapat faktor human error yang berupa kelalaian dan keteledoran operator mesin percetakan sehingga terjadinya ketidaksesuaian target produksi dengan output produksi yang telah ditentukan oleh perusahaan. Pada penelitian ini untuk mengetahui faktor yang berpengaruh terhadap human error serta peluang terjadinya kegagalan proses percetakan digunakan metode Hierarchical Task Analysis (HTA). HTA digunakan untuk melakukan breakdown tiap task pada proses percetakan serta menganalisis tiap task sehingga dapat mengetahui task yang bisa mempengaruhi target produksi yang tidak tercapai. Dalam peningkatkan pencapaian target produksi dilakukan penilaian Human Error Probability (HEP) terjadinya kesalahan pada operator mesin percetakan dengan metode Human Error Assessment and Reduction Technique (HEART) sehingga dapat mengetahui besar peluang terjadinya human error serta human reliability pada tiap proses percetakan. Pada metode HEART terdapat penentuan nilai APOE secara subjektivitas melalui pendapat expert judgment. Untuk mengurangi penilaian subjektivitas digunakan modifikasi penentuan nilai APOE dengan Fuzzy Lingustic Expression. Hasil dari penelitian ini dengan metode HTA dan HEART didapatkan bahwa nilai human reliability operator percetakan secara keseluruhan tergolong sangat rendah hanya sebesar 0,313 dengan nilai keandalan operator tiap proses percetakan pada bagian printing 0,786, bagian hotprint 0,906, bagian UV coating 0,712, bagian die cutting 0,661 dan bagian folder gluer 0,928. Dari hasil penelitian tersebut selanjutnya dilakukan pemecahan masalah dengan mencari akar penyebab terjadinya masalah tersebut dengan salah satu metode Root Cause Analysis (RCA) yaitu menggunakan metode 5 Why Method sehingga didapatkan rekomendasi perbaikan yang diharapkan dapat mengurangi permasalahan yang terjadi pada perusahaan.
English Abstract
CV. Aneka Grafika is one of companies production in printing packaging cardboard. The problem that often occurs in the company is unreachable production target which can hamper its operations. This happens because there are human error factors such as negligence and carelessness from the printing machine operators which is resulting in the mismatch of the production target and output production set by the company. This research is conducted to determine factors that impact on human error and the probability of failure on printing process using method of hierarchical task analysis (HTA). HTA is used to breakdown every task of the printing process and analyze every task, so that can know the task that will influence the unachieved production target. In order to increase the achievement of production target, Human Error Probability (HEP) assessment is done by calculating the errors from the printing operators using the Human Error Assessment and Reduction Technique (HEART) method so the probability of human error and human reliability can be known on every printing process. On the HEART method, there is the assessment of APOE value in subjectivity through expert judgement opinion. The modification of the determination of APOE value using Fuzzy Linguistic Expression is used to reduce subjectivity assessment. The result of this research using the HTA and HEART method is obtained that the overall human reliability of printing operators were very low, only 0,313 with the human reliability of the operators from every printing process, on the printing 0,786, the hot print 0,906, the UV coatings 0,712, the die cutting 0,661, and the folder gluer 0,928. From the results of the research, then do the troubleshooting by finding the root cause of this problem using one of the Root Cause Analysis (RCA) method that is 5 Why method so the recommendations for improvements can be known for reducing the problems that happen to the company.
