Abstract

Latar Belakang : Kolestasis adalah kegagalan aliran cairan empedu masuk duodenum dalam jumlah normal. Stasis cairan empedu pada sistem bilier intrahepatal menyebabkan jejas pada sel-sel hepar berakibat nekrosis sel hepar yang ditandai release aspartate aminotransferase (AST) dan alanin aminotransferase (ALT). Nekrosis pada parenkim hepar menyebabkan perubahan struktur anatomis vena porta yang berpengaruh terhadap diameter dan kecepatan aliran vena porta. Tujuan dari studi ini adalah untuk mengevaluasi hubungan rasio AST/ALT dengan diameter dan kecepatan aliran vena porta pada penderita kolestasis anak. Metode : jenis penelitian observasional analitik dengan cross sectional study, menggunakan consecutive tehnik sampling. Rasio AST/ALT sebagai variable independent dengan ketentuan AST/ALT 1. Sebagai variabel dependen diameter dan kecepatan vena porta yang dilukur dengan USG B mode dan color Doppler. Analisis hubungan antar variabel dilakukan dengan korelasi Pearson menggunakan derajat kepercayaan 95%. Hasil: Didapatkan 14 penderita (laki-laki 71,4 %; usia rata-rata 35,4 hari) . Pada penderita anak dengan kolestasis hanya didapatkan hubungan bermakna antara rasio AST/ALT dengan diameter vena porta (p=0.012 r =-0,648). Kesimpulan : Semakin rendah rasio AST/ALT maka semakin lebar diameter vena porta.

English Abstract

Background. Cholestasis is failure of billiary flow into duodenum in normal amount. Stasis of bile in intrahepatic biliary system may cause injury to liver cells resulted in marked hepatic cell necrosis which release aspartate aminotransferase (AST) and alanine aminotransferase (ALT). Necrosis of liver parenchyma causes anatomical distortion that affect portal vein diameter and flow velocity. Methods. purpose of this study was to evaluate relationship of AST/ALT ratio with portal vein diameter and flow velocity in cholestatic children. relationship between variables was analyzed with Pearson and Chi-Square test. Results. 14 patients (71.4% male, mean age 35.4 days) were diagnosed with cholestasis and AST/ALT ratio 1. B-mode and Color Doppler sonography was performed on each patient to evaluate portal vein diameter and flow velocity. re is a significant relationship between AST/ALT ratio with portal vein diameter (p = 0,012 r=-0,648) in cholestatic children. Conclusion. Dilated portal vein diameter is associated with lower AST/ALT ratio in cholestatic children.
