Abstract

Latar Belakang. Kanker kolorektal merupakan salah satu keganasan yang cukup tinggi insidens-nya didunia dan menduduki urutan ke-3. Peningkatan ekspresi cyclooxygenase-2 (COX-2) akan menginduksi angiogenesis, sehingga dikatakan bahwa COX-2 mempunyai peran dalam pertumbuhan kanker kolorektal. Ekspresi COX-2 dalam keadaan normal akan disupresi oleh p53 wild type, apabila terjadi mutasi p53 maka akan terjadi peningkatan ekspresi COX-2. Tujuan. Mengetahui ekspresi cyclooxygenase-2 dan p53 mutan serta korelasi antara cyclooxygenase-2 dengan p53 mutan pada kanker kolorektal. Metode. Penelitian survei dan data diambil secara cross sectional. Data dianalisa secara diskriptif analitik untuk mencari korelasi antara cyclooxygenase-2 dengan p53 mutan pada kanker kolorektal. Hasil. Di antara 20 pasien kanker kolorektal, 10 pasien (50%) laki-laki dan 10 pasien (50%) perempuan. Umur (mean ) 50,15±12,99 (21-70 tahun). Distribusi lokasi kanker kolorektal, 13 pasien (65%) di rektum, 2 pasien (10%) di sigmoid, 2 pasien (10%) di kolon asenden, 1 pasien (5%) di rektosigmoid, 1 pasien (5%) di kolon transversum dan 1 pasien (5%) di sekum. Hasil histopatologi terbanyak low grade 18 pasien (90%) dan high grade sebanyak 2 pasien (10%). Ekspresi COX-2 positif didapatkan sebanyak 17 pasien (85%), terdiri dari COX-2 positif pada laki-laki sebanyak 8 pasien (40%) dan pada perempuan sebanyak 9 pasien (45%). Ekspresi p53 mutan positif didapatkan pada 13 pasien (65%), terdiri dari p53 mutan positif pada laki-laki sebanyak 5 pasien (25%) dan pada perempuan sebanyak 8 pasien (40%). Nilai koefisien Spearman untuk korelasi antara COX-2 dengan p53 mutan sebesar 0,692 dengan nilai signifikansi (p) sebesar 0,001. Kesimpulan. Pada kanker kolorektal didapatkan ekspresi COX-2 dan p53 mutan dan keduanya berkorelasi secara signifikan.

English Abstract

Background: Colorectal cancer is one of high incidences of malignancy of his world and dropped to number three. Increased expression of cyclooxygenase-2 (COX-2) will induce angiogenesis, so it is said that COX-2 has a role in colorectal cancer growth. COX-2 expression in normal circumstances would suppression by wild-type p53, p53 mutation occurs when re will be increased expression of COX-2. Objective: To examine expression of cyclooxygenase-2 and p53 mutant and correlation between cyclooxygenase-2 with p53 mutant in colorectal cancers. Methods: Survey research and data collected by cross sectional study. Data were analyzed descriptively, analytically to find correlation between cyclooxygenase-2 with p53 mutant in colorectal cancers. Results: Among 20 patients with colorectal cancer, 10 patients (50%) male and 10 patients (50%) women. Aged (mean) 50.15 ± 12.99 (21-70 years). Location distribution of colorectal cancer, 13 patients (65%) in rectum, two patients (10%) in sigmoid, two patients (10%) in ascending colon, one patient (5%) in recto sigmoid, one patient (5%) in colon transverse and one patient (5%) in secum. Most low-grade histopathology results of 18 patients (90%) and high grade as much as two patients (10%). COX-2 positive expression was found as many as 17 patients (85%), consisting of COX-2 positive in males as many as eight patients (40%) and in women as many as nine patients (45%). p53 mutant positive expression was found in 13 patients (65%), consisting of p53 mutant positive in males as many as five patients (25%) and in women as many as eight patients (40%). Spearman coefficient for correlation between COX-2 with p53 mutant at 0.692 with a significance value (p) equal to 0.001. Conclusion: In colorectal cancers showed expression of COX-2 and p53 mutant and both correlated significantly.
