Abstract
Latar Belakang. / Pendekatan terapi nefropati diabetes saat ini adalah melakukan deteksi secara dini,(adanya mikroalbuminuria). Paradigma baru adalah memprediksi dan pencegahan terjadinya mikroalbuminuria (pencegahan primer). 1 Hal ini karena berkembangnya teknologi genomik dan proteomik dengan pengenalan biomarker (urin, plasma) dan petanda molekuler (biopsi). Kekacauan MMP-9, (aktivitas proteolitik lokal) diperkirakan faktor yang penting terjadinya kerusakan barier filtrasi glomerulus yang mengakibatkan terjadinya proteinuria. 4,5 / Tujuan . Untuk mengetahui korelasi antara kadar MMP-9 serum dengan kadar Albuminuria pasien Diabetes Mellitus tipe 2. Metode. Diteliti 52 subyek DM tipe 2 dan 18 orang non DM, diagnosa DM menurut kriteria ADA. Kadar MMP-9 serum diukur dengan metode ELISA. Subyek penelitian digolongkan 3 kelompok berdasarkan kadar ACR: normoalbuminuria (26 orang), mikroalbuminuria (23 orang), dan makroalbuminuria (3 orang). Hasil . Rata-rata kadar MMP-9 serum subyek diabetes (24.33 ± 16.86) signifikan lebih tinggi dibanding orang normal (6.49 ± 0.64.),dengan p 0.001. Rata-rata kadar MMP-9 serum subyek DM normoalbuminuria tidak berbeda bermakna dengan kelompok mikroalbuminuria (p=0.058) tetapi berbeda bermakna bila dibandingkan kelompok makroalbuminuri (p 0.001). Didapatkan hubungan yang signifikan antara kadar MMP-9 serum dengan kadar ACR pada subyek DM (r=0.64, p 0.001). Kesimpulan. Kadar MMP-9 serum penderita DM tipe 2 lebih tinggi bila dibanding orang non DM. Pada DM tipe 2, terdapat hubungan yang signifikan antara kadar MMP-9 serum dengan kadar albuminuria. Semakin tinggi kadar MMP-9 serum semakin tinggi kadar ACR.
English Abstract
Background . Diabetic nephropathy rapy approach now is to do early detection, ( existence of microalbuminuria). new paradigm is to predict and prevent occurrence of microalbuminuria (primary prevention). 1 This is because development of genomics and proteomics technology with introduction of biomarkers (urine, plasma) and molecular markers (biopsy). Local proteolytic activity caused by MMP-9, predicted that an important factor glomerular filtration barrier damage which resulted in a proteinuria. 4,5 Purpose . To find out correlation between levels of MMP-9 serum levels of albuminuria of patients with Type 2 Diabetes Mellitus. Methods. Studied 52 subjects with type 2 diabetes mellitus and 18 non-DM, diagnosis of DM according to ADA criteria. MMP-9 serum levels were measured with ELISA method. 3 groups of study subjects classified according to levels of ACR: normoalbuminuria (26 people), microalbuminuria (23 people), and macroalbuminuria (3 people). Results . Average serum levels of MMP-9 diabetic subjects (24.33 ± 16.86) significantly higher than normal people (6:49 ± 0.64.), with p 0.001. Average serum levels of MMP-9 in normoalbuminuria subjects did not differ significantly with microalbuminuria group (p = 0.058) but was significantly different when compared macroalbuminuria group (p 0.001). Found a significant relationship between serum levels of MMP-9 with ACR levels in diabetic subjects (r = 0.64, p 0.001). Conclusion . Levels of MMP-9 serum patients with type 2 DM was higher when compared to non-DM. In type 2 diabetes, have a significant relationship between levels of serum MMP-9 and levels of albuminuria. levels of serum MMP-9 in patients with ND increased in accordance with clinical stage of disease.
