Abstract

Latar Belakang. Parasetamol, antipiretik analgesik yang banyak beredar di pasaran memiliki efek samping toksisitas hati akibat pembentukan radikal bebas yang menyebabkan nekrosis hepatosit. Pencegahan efek toksik obat pada hati telah diteliti melalui berbagai studi klinis dengan pemberian silimarin dan lesitin. Genistein adalah zat aktif isoflavon yang banyak terkandung dalam kedelai. Pemberian genistein peroral mampu mengurangi peroksidasi lemak di hati dan meningkatkan kapasitas total antioksidan. Tujuan . Meneliti kelainan biokimia, histopatologi dan membandingkan efek pemberian genistein, silimarin dan lesitin, dalam memperbaiki nekrosis sel hati rattus novergicus Strain Wistar yang diberi parasetamol dosis toksik. Metode. Penelitian eksperimental laboratorik pada 48 tikus wistar jantan, terbagi 4 kelompok.Kelompok I, kelompok kontrol yang diberikan Parasetamol dosis 600mg/kgBB peroral. Kelompok II Parasetamol 600mg/kgBB plus Genistein 2 mg/kgBB. Kelompok III mendapatkan Parasetamol 600mg/kgBB plus Silimarin 50mg/kgBB. Kelompok IV mendapatkan Parasetamol 600mg/kgBB plus Lesitin 100 mg/kgBB. Parameter yang diukur adalah kadar AST, ALT, Asam empedu serta kadar Malondialdehida (MDA) dan Glutation (GSH). Histopatologi hepatosit diperiksa dengan pewarnaan HE untuk melihat nekrosis sentrilobuler. Hasil . Kadar AST, ALT lebih rendah signifikan pada kelompok II yang mendapat genistein (p=0.004 dan p=0.000). Kadar asam empedu terendah pada kelompok yang mendapat lesitin (p=0.025). Kadar MDA terendah didapatkan pada kelompok III yang mendapat silimarin (p=0.009). Kadar GSH tertinggi pada kelompok IV yang mendapat lesitin (p=0.000). Persentase nekrosis sentrilobuler terendah pada kelompok yang mendapat genistein (p=0.000). Kesimpulan . Pemberian genistein, silimarin dan lesitin memperbaiki nekrosis hati akibat parasetamol dosis toksik. Genistein paling signifikan dalam memperbaiki nekrosis sel hati.

English Abstract

Background : Paracetamol, an antipyretic analgesic that is distributed widely in market has side effect of liver toxicity due to formation of free radicals that cause necrosis of hepatocytes. Prevention of toxic effects of drugs on liver had been studied through a variety of clinical trial with administration of herbal medicine such as silymarin and lecithin. Isoflavone genistein is an active substances which are found mainly in soybeans. Oral administration of genistein reduces lipid peroxidation in liver and increase total antioxidant capacity. Objectives: To examine biochemical, histopathology abnormalities and compare effects of genistein, silymarin and lecithin in improving liver necrosis of rattus novergicus Wistar strain induced by toxic doses of paracetamol. Methods: Experimental laboratory studies on 48 male wistar rats, y were divided into 4 groups. Group I, control group given a dose paracetamol peroral 600mg/kgBWB. Group II Paracetamol 600mg/kgBW plus Genistein 2 mg / kg. Group III 600mg/kgBB Paracetamol plus 50mg/kgBBW Silymarin. Group IV 600mg/kgBB Paracetamol plus Lecithin 100 mg/kgBW. Measured parameters were levels of AST, ALT, bile acid, Malondialdehida (MDA) and Glutathione (GSH). Centrilobuler necrosis detected in histopathology examination using HE staining. Results: Levels of AST, ALT was significantly lower in group II which received genistein (p=0.004 and p=0.000). Bile acid level was lowest in group receiving lecithin (p=0.025). Lowest tissue MDA level obtained in group III which received silymarin (p=0.009). highest GSH level in group IV that received lecithin (p=0.000). Lowest percentage of centrilobuler necrosis was measured in group receiving genistein (p=0.000). Conclusion: A dministration of genistein, silymarin and lecithin improve liver necrosis induced by toxic doses of paracetamol. Genistein is most powerful in enhancing liver regeneration.
