Abstract

Latar Belakang: Defisiensi vitamin D dihipotesiskan berperan dalam peningkatan insiden asma. Peran vitamin D pada asma sebagai imunomodulator, bekerja pada sel dendritik dan sel T mempromosikan Treg dalam mensekresi IL-10. Saat ini penelitian mengenai vitamin D dan asma belum banyak diteliti pada subjek dewasa dan belum diteliti di Indonesia. Tujuan penelitian ini untuk membuktikan hubungan antara kadar plasma vitamin D dengan tingkat kontrol, fungsi paru, kadar plasma IL-10 dan IL-17 pada penderita asma bronkial persisten. Metode: Pada studi cross sectional ini, 38 penderita asma bronkial persisten di Poliklinik Paru, RSU dr. Saiful Anwar Malang diukur kadar plasma vitamin D, tingkat kontrol asma (skor ACT), fungsi paru (VEP1), kadar plasma IL-10 dan IL-17. Pengukuran kadar vitamin D, IL-10 dan IL-17 dengan metode ELISA. Hasil: Rerata kadar plasma vitamin D adalah 15,87 ± 3,31 nmol/L. Seluruh pasien (100%) mengalami defisiensi vitamin D. Kadar plasma vitamin D tidak berhubungan signifikan dengan tingkat kontrol asma (p = 0,560) dan fungsi paru (p = 0,845). Kadar plasma vitamin D berhubungan negatif secara bermakna terhadap kadar IL-10 (r = -0,339, p = 0,039) dan IL-17 (r = -0,328, p = 0,045). Kesimpulan: Kadar vitamin D berhubungan negatif dengan kadar IL-10 dan IL17. Kadar vitamin D tidak berhubungan dengan tingkat kontrol asma dan fungsi paru.

English Abstract

Background: Vitamin D deficiency is hypo sized to play a role in increasing incidence of asthma. Vitamin D has a role as an immunomodulator in asthma. Current research on vitamin D and asthma hasn`t been studied in adult subjects in Indonesia. aim of this study was to investigate association between plasma vitamin D level with asthma control level, pulmonary function, plasma IL-10 and IL-17 level in persistent asthma patients. Method: In this cross sectional study, 38 patients with persistent asthma were enrolled from Pulmonary Clinic, dr. Saiful Anwar General Hospital Malang to asses relationship between plasma vitamin D level with asthma control level (ACT score), pulmonary function (FEV1), plasma IL-10 and IL-17 level. Vitamin D, IL-10 and IL-17 level were measured by ELISA. Result: mean plasma vitamin D level was 15.87±3.31 nmol/L. All patients (100%) had vitamin D deficiency. Plasma vitamin D level was not significantly associated to asthma control level (p=0.560) and FEV1 (p=0.845). Plasma vitamin D levels was significantly negative associated to IL-10 (r=-0.339, p=0.039) and IL-17 level (r=-0.328, p=0.045). Conclusion: Vitamin D level was inversely associated to IL-10 and IL17 level. Vitamin D level was not associated to asthma control level and pulmonary function.
