Abstract

Infeksi HIV berhubungan erat dengan TB MDR. Koinfeksi HIV dan TB MDR menimbulkan tingginya angka mortalitas. suPAR merupakan penanda biologis yang bisa menilai status imun penderita. Saat ini belum diketahui apakah kadar suPAR memiliki informasi untuk memprediksi TB MDR pada TB HIV. Tujuan penelitian ini untuk mengevaluasi peranan kadar suPAR dalam memprediksi TB MDR pada koinfeksi TB HIV. Metode. 15 TB HIV, 15 TB MDR dan 10 kontrol dilibatkan. Sampel darah diambil untuk pengukuran kadar suPAR dengan ELISA dan sputum dikirim kultur dan sensitivitas OAT. Hasil. Nilai rerata kadar suPAR pada TB HIV (15,26+5,37) lebih tinggi dari TB MDR (7,75+2,64) dan kontrol sehat (1,76+0,57) dengan hasil perbedaan bermakna (p = 0,000). Tidak ada perbedaan bermakna antara kadar suPAR pada TB HIV kultur positif dan kultur negatif (p = 0,138), begitu juga kadar suPAR pada TB HIV BTA positif dan BTA negatif (p = 0,058). Kesimpulan. Kadar suPAR pada TB HIV secara bermakna lebih tinggi dibanding TB MDR. Kadar suPAR pada TB HIV belum bisa dijadikan biomarker untuk memprediksi TB MDR. Kadar suPAR bisa digunakan untuk memprediksi kemungkinan koinfeksi TB HIV.

English Abstract

HIV infection relates to MDR TB closely. HIV-MDR TB coinfection causes high mortality rate. suPAR is a biological marker which can evaluate patient`s immune state. At this time it`s still unknown whether suPAR level has information to predict MDR TB in TB HIV patients. This study`s goal was to evaluate the role of suPAR level in predicting MDR TB in TB HIV coinfection. Method: 14 TB HIV patients, 14 MDR TB patients and 10 controls were enrolled. Blood samples were collected to measure suPAR level using ELISA. Sputum spesimens were send for culture and OAT sensitivity test. Result: Average of suPAR level in TB HIV patients (15,26+5,37) is higher than in MDR TB (7,75+2,64) and in control (1,76+0,57) with significantly difference (p = 0,000). There`s no significantly difference between suPAR level in positive culture and negative culture TB HIV patients (p=0,138), nor between positive smear AFB and negative smear AFB TB HIV patients (p=0,058). Conclusion: suPAR level in TB HIV is significantly higher compared with MDR TB. This makes us couldn`t use it as biomarker to predict MDR TB. The higher suPAR level can be used to predict TB-HIV coinfection.
