Abstract

Diabetes Melitus tipe 2 (DMT2) merupakan penyakit gangguan metabolik yang disebabkan oleh resistensi insulin dan penurunan fungsi sel β pankreas. Peningkatan ROS (Reactive Oxygen Species) pada DMT2 dapat menyebabkan komplikasi ke berbagai organ, salah satunya adalah ginjal. Treadmill merupakan salah satu upaya untuk mengurangi efek DMT2 dengan memperbaiki homeostasis glukosa dan meningkatkan sensitifitas insulin. Penelitian ini bertujuan untuk mengetahui pengaruh treadmill terhadap kadar malondialdehid (MDA) dan gambaran histopatologi ginjal pada tikus (Rattus norvegicus) model DMT2 dengan induksi high fat diet (HFD) sebanyak 40 g/ekor/hari selama 64 hari dan injeksi multiple low dose streptozotocin (MLD STZ) dosis 30 mg/kg BB secara intraperitoneal, sebanyak dua kali injeksi dengan interval pemberian injeksi 1 minggu. Penelitian ini menggunakan tiga kelompok yaitu normal sedentari, DMT2 sedentari, dan DMT2 treadmill. Treadmill dilakukan selama 6 hari dalam seminggu, dengan kecepatan dan durasi yang meningkat setiap 2 minggu yaitu 10 meter/menit selama 10 menit, 14 meter/menit selama 22 menit, 18 meter/menit selama 34 menit, 22 meter/menit selama 46 menit, dan 27 meter/ menit selama 60 menit. Kadar MDA diukur dengan menggunakan metode Thiobarbituric Acid (TBA) dan gambaran histopatologi ginjal diwarnai dengan Hematoxylin Eosin (HE). Hasil penelitian menunjukkan treadmill secara signifikan (p<0,05) menurunkan kadar MDA dan menurunkan ekspansi sel mesangial, penebalan membran filtrasi serta memperbaiki kerusakan sel-sel ginjal, dengan cara meningkatkan ambilan glukosa dan sensitivitas insulin, sehingga dapat disimpulkan bahwa latihan fisik treadmill dapat menurunkan kadar MDA dan memperbaiki histopatologi ginjal pada tikus model DMT2.

English Abstract

Type 2 diabetes mellitus (T2DM) is a metabolic disorder caused by retention of insulin and reduction of pancreatic β-cell function. Increased ROS (Reactive Oxygen Species) in T2DM can cause complication to various organs, such as kidney. Treadmill is one attempt to reduce the effects of T2DM by improving glucose homeostasis and insulin sensitivity. The aims of this study was to determine the effect of treadmill towards malondialdehyde (MDA) level and the description of kidney histopathology on T2DM rats (Rattus norvegicus) induced by high fat diet (HFD) 40 g/rat/day for 64 days and multiple low dose streptozotocin (MLD STZ) injection of 30 mg/kg BW intraperitoneally, given twice at interval a week administration. The rats were divided into three groups: normal sedentary, T2DM sedentary, and T2DM treadmill. The treadmill was conducted for 6 days in a week, with the speed and duration increased every 2 weeks are 10 meters/minute for 10 minutes, 14 meters/minute for 22 minutes, 18 meters/minute for 34 minutes, 22 meters/minute for 46 minutes and 27 meters/minute for 60 minutes. MDA level were determined using Thiobarbituric Acid (TBA) Assay and histopathological observation of kidney stained using Hematoxylin Eosin (HE). The result showed that physical exercise (treadmill) decrease MDA level significantly (p<0,05) and decrease mesangial expansion, filtration membrane thickness, and repaired kidney cells damage, by increasing insulin independent glucose uptake and insulin sensitivity. The conclussion is physical exercise (treadmill) decrease MDA level and repaired histopathological kidney in rat model of T2DM.
