Abstract

Inflammatory Bowel Diseases (IBD) is a merupakan inflamasi yang menyerang saluran pencernaan yaitu lambung, usus halus dan kolon. Golongan Non-Steroidal Antiinflamatory Drugs (NSAID) merupakan salah satu penyebab terjadinya keparahan IBD. Ekstrak metanol daun Kamboja putih (Plumeria acuminata) yang mengandung lupeol asetat digunakan sebagai antiinflamasi dan antioksidan alami. Tujuan penelitian ini adalah untuk mengetahui pengaruh pemberian terapi ekstrak daun kamboja putih (Plumeria acuminata L) terhadap perubahan histopatologi dan ekspresi cyclooxygenase-2 (COX-2) organ duodenum pada hewan model tikus IBD. Tikus model IBD dihasilkan dengan cara pemberian indometasin 15 mg/kg BB secara per oral. Penelitian ini menggunakan lima kelompok tikus (Rattus norvegicus), yaitu kelompok kontrol negatif, kelompok kontrol positif (IBD), kelompok terapi dosis 500 mg/kg BB, 750 mg/ kg BB dan 1000 mg/kg BB. Pengamatan histopatologi organ duodenum menggunakan mikroskop dan pengukuran ekspresi cyclooxygenase-2 (COX-2) dilakukan dengan metode Immunohistokimia (IHK) dan diukur menggunakan program immunorasio kemudian dianalisis dengan menggunaan tes ANOVA dengan α = 0,05. Hasil penelitian menunjukkan bahwa pemberian ekstrak daun kamboja putih mampu memperbaiki kerusakan mukosa duodenum serta menurunkan infiltrasi sel radang pada tikus model IBD serta menurunkan persentase area ekspresi COX-2 secara signifikan (p<0,05) antar kelompok perlakuan. Kelompok dengan dosis terapi 1000 mg/kg BB adalah dosis efektif menurunkan ekspresi COX-2 sebesar 68,62%. Disimpulkan bahwa ekstrak metanol daun kamboja putih dapat digunakan sebagai terapi IBD.

English Abstract

Inflammatory Bowel Disease (IBD) is a digestive inflammatory disease that attacks the stomach, small intestine and large intestine. Non-Steroidal Antiinflamatory Drugs (NSAID) group is one of IBD caused. The methanolic extract of Plumeria acuminata leaf contain of lupeol acetate used as antiinflammatory and antioxidant. The purpose of the research was to study the potency of methanolic extract of Plumeria acuminata leaf that could repair duodenal histopathology and decrease cyclooxygenase-2 (COX-2) expression on IBD rats. This research used Rats (Rattus norvegicus) Wistar strain aged between 8-12 weeks old and had average of 150-200 g of BW. IBD Rats were prepared by administered of 15 mg/kg BW indomethacin, orally. There were five groups of rats used in this research: control group (A); IBD group (B) and three groups with therapy of methanolic extract of Plumeria acuminata leaf dose of 500 mg/kg BW (C), 750 mg/kg BW (D) and 1000 mg/kg BW (E). Duodenal Histopathology were observed microscopically and COX-2 expression was observed by immunohistochemistry method and measured by Imunoratio software. Data were analyzed with ANOVA test followed by Honestly Significant Difference (HSD) test (α = 0.05). The result showed that methanolic extract of Plumeria acuminata leaf could repair mucosal epithelial duodenal cells from erotion and decrease infiltration of inflammatory cells and also decrease COX-2 espression, significantly (p< 0.05). Dose of 1000 mg/kg BW is the best result to decrease COX-2 expression up to 68.62%. The conclusion was Plumeria acuminata leaf extract could be used as IBD theraphy.
