Abstract

Gagal ginjal stadium terminal memiliki prevalensi yang cukup tinggi di Indonesia. Adekuasi dialisis merupakan prediktor independen terhadap morbiditas dan mortalitas pasein CAPD. Tujuan penelitian ini untuk mengukur mengetahui faktior-faktor yang berkaitan dengan adekuasi peritoneal dialisis. Penelitian ini menggunakan studi cross sectional dengan startified random sampling. Adekuasi ditentukan dengan menghitung weekly Kt/V dan weekly creatinine clearance. Baik wKt/V dan wCCr dihubungkan dengan eGFR, lama pengunaan CAPD, status diabetes, riwayat peritonitis, luas permukaan tubuh, kecepatan transpor membran dan volume urine. Analiis data menggunakan statistik Pearson dan Point Biserial. Hasil: 68 pasien CAPD dengan karakteristik luas permukaan tubuh (1,60 ±0,19) m2, riwayat peritonitis (16,2%), diabetes mellitus (33,8%), lama penggunaan CAPD (24,47 ± 29,20) bulan, dan produksi urine 24 jam (289,41 ± 371,76) mL. Karakteristik transpor membran terbesar adalah high average (51,5%), sementara adekuasi wKt/V adalah 1,84±0,56 L/minggu/1,73m2 dan wCCr 61,51 ± 23,69 L/minggu. Ditemukan korelasi signifikan antara wCCr dengan EGFR (r = 0,371, p = 0,002), BSA (r = -0,303, p = 0,012), transport membran peritoneal (r = 0,063, p = 0,041) dan produksi urine (r = 0,379, p = 0,001). Serta hubungan yang signifikan terhadap adekuasi wKt/V yaitu BSA (r = -0,571, p = 0,000), transpor membrane peritoneal (r = 0,224, p = 0,046), dan produksi urin (r = 0,243, p = 0,046). Dapat disimpulkan rerata pasien CAPD dalam penelitian ini telah memenuhi target adekuasi. Pada penelitain ini terdapat hubungan yang signifikan antara adekuasi dengan eGFR, produksi urine, luas permukaan tubuh dan transpor membran peritoneal

English Abstract

Prevelency end stage renal disease is increasing in Indonesia. Dialysis adequacy remains an independent predictor of morbidity and mortality in CAPD patients. This study aimed to measure adequacy and to identify several factors that related to the adequacy of CAPD patients. Cross sectional study with stratified random sampling was used in this study. Adequacy were determined by measuring weekly Kt/V and weekly creatinine clearance. Both wKt/V and wCCr were analyzed its correlation to eGFR, duration of CAPD, diabetic status, history of peritonitis, body surface area, membrane transport characteristics and urine volume. Data were analyzed using Pearson and Point Biserial correlation test. Results: 64 CAPD patients had characteristics BSA (1.60±0.19m2), peritonitis history (16.2%), diabetes mellitus (33.8%), duration of CAPD (24.47±29.20months), 24h urine production (289,41±371.76mL). Membrane transport characteristics mostly were low average (36.8%) and high average (51.5%), whether adequacy wKt/V was 1.84±0.56 L/week/1.73m2 and wCCr 61.51±23.69. We found significant correlation between wCCr with EGFR (r = 0.371, p = 0.002), BSA (r = -0.303, p = 0.012), variabel membrane transport (r = 0.063, p = 0.041) and urine production (r = 0.379, p = 0,001). Significant correlation between wKt/V with BSA (r = -0.571, p = 0.000), membrane transport (r = 0.224, p = 0.046), and urine production (r = 0.243, p = 0.046). In average, CAPD patients in this study met sufficent adequacy. There is a significant correlation between adequacy with eGFR , urine production , body surface area and membrane transport
