Abstract
Di Indonesia, Lupus Eritematosus Sistemik (LES) adalah suatu penyakit autoimun yang penyebabnya belum sepenuhnya diketahui sedangkan perjalanan klinisnya beragam sehingga disebut ‘Penyakit Seribu Wajah’. Sekitar 50% pasien penderita LES jatuh pada kondisi Lupus Nefritis. Lupus nefritis adalah gangguan ginjal yang diakibatkan produksi autoantibodi berlebih terhadap antigen sendiri sehingga menyebabkan kerusakan organ dan berakibat kematian. Telah ditemukan bukti bahwa protein HMGB-1(High Mobility Group Box-1) berkontribusi pada patogenesis inflamasi dan penyakit autoimun. Ekskresi HMGB-1 pada urin merefleksikan inflamasi ginjal pada LES. penelitian ini mengukur kadar HMGB-1 pada urin penderita LES diharapkan memeliki korelasi terhadap klinis dan biokimia dari ginjal dan aktifitas penyakit sistemik. Penelitian ini menggunakan ekperimental murni yang bertujuan untuk membuktikan bahwa antibody anti HMGB-1 dapat digunakan untuk mendeteksi komplikasi lupus nefritis pada pasien LES. Sampel penelitian adalah urin pasien LES yang yang telah didiagnosis oleh dokter spesialis pemyakit dalam konsultan rheumatologi di RS Dr. Saiful Anwar, Malang sebanyak 5 kategori pasien normal,pasien LES, pasien Silent Lupus Nefritis dan pasien LES dengan Lupus Nefritis. Protein dalam urin pasien diisolasi dan dicari protein HMGB-1 30 kDa. Hasil densitas HMGB-1 dengan protein urin, indeks aktivitas, dan indeks kronisitas menunjukkan bahwa terjadi korelasi yang signifikan dengan semakin tebalnya densitas HMGB-1 maka semakin tinggi pula kadar proteinurin pada sampel urin pasien.
English Abstract
In Indonesia, Systemic Lupus Erythematosus (LES) is an autoimmune disease which the cause is still not fully unknown, while natural history of Nephritic Lupus is varied. Nephritic Lupus is a kidney disorder caused by excessive production of auto antibodies against self-antigens that cause organ damage and result in death. Research found evidence that HMGB protein-1 (High Mobility Group Box-1) contributes to the pathogenesis of inflammatory and autoimmune diseases. HMGB-1 excretion in urine reflects renal inflammation in the LES. This study measured levels of HMGB-1 in urine LES patients are expected to have a correlation to clinical and biochemical activity of the kidney and systemic diseases. This pure experimental study aimed to prove that the anti-HMGB-1 antibody can be used to detect complications of nephritic lupus in patients with LES. Samples were urine of LES patients who had been diagnosed by a medical specialist in rheumatology consultant at the hospital Dr. Saiful Anwar Malang divided by five categories of normal patients, patients with LES, Silent Nephritic Lupus patients and patients with Nephritic Lupus LES. Protein in the urine of patients isolated and researcher detects the amount of HMGB protein-1 30 kDa. Density result HMGB-1 with urinary protein, activity index, and chronicity indices show that there is a significant correlation with the thick density of HMGB-1, the higher the protein content in the urine urine samples of patients.
