Abstract
Berdasarkan SDKI tahun 2012 sebanyak 9% tingginya AKI disebabkan persalinan lama dengan faktor resiko stres dalam persalinan. Penelitian ini bertujuan untuk mendapatkan gambaran kelancaran persalinan kala satu fase aktif pada subjek penelitian yang mengikuti program Darul Quran Sehat. Penelitian menggunakan metode deskriptif dengan analisis data univariat untuk mendapatkan gambaran dan distribusi karakteristik frekuensi yang diteliti. Pengambilan data sampel 56 orang terdiri dari 21 orang primipara dan 35 orang multipara dengan menggunakan data rekam medik pasien bersalin di Klinik Darul Quran Sehat, Malang. Hasil penelitian dari 56 orang subjek penelitian menunjukkan dilatasi serviks lancar 80% pada kelompok primipara dan 69% pada kelompok multipara. Penyebab dilatasi serviks tidak lancar pada subjek penelitian yang rutin maupun tidak rutin melaksanankan program Darul Quran Sehat disebabkan adanya faktor penyulit persalinan seperti perineum kaku, janin besar, dan kontraksi uterus yang tidak adekuat. Kesimpulan yang didapatkan kelancaran dilatasi serviks kala satu fase aktif pada kelompok primipira maupun multipara tidak hanya dipengaruhi oleh faktor kerutinan melaksanakan program Darul Quran Sehat melainkan dipengaruhi oleh faktor perineum kaku (passage), inadekuat kontraksi uterus (power), dan janin besar (passenger) yang dapat mengakibatkan partus lama. Implikasi kebidanan, bidan dapat memberikan pelayanan persiapan proses persalinan lebih awal dengan memperhatikan penyulit persalinan melalui program Darul Quran Sehat. Penelitian ini tidak meneliti kuat hubungan pelaksanaan program Darul Quran Sehat terhadap kelancaran persalinan kala satu fase aktif sehingga penelitian selanjutnya diharapkan untuk meneliti hal tersebut.
English Abstract
Based on Demography and Health Indonesia Survey in 2012 as much as 9% high maternal mortality rate due to long labor and their risk factors of stress in labor. This research aims to get a description of a continuity delivery in first active phase on the research subject who was follow the Darul Quran Sehat’s program. This research using descriptive method univariate data analysis to get an overview and observed the characteristic distribution frequencies. Data retrieval 56 sample consisting of 21 nuliparous and 35 multiparous people by using medical record data of patients in the maternity clinic at the Darul Quran Sehat, Malang. Research results of the 56 people shows 80% of the nuliparous and 69% at multiparous group having cervical dilation smoothly. Cause of cervical dilatation unsmoothly on the research subject who was implement Darul Quran Sehat’s program routine and non-routine due to labor complications factors such as rigid perineum, birthing giant baby, and inadequate uterine contractions. Conclusions obtained which cervical dilatation smoothly at first active phase on nuliparous and multiparous group not only influenced by implementing Darul Quran Sehat’s program routinely but also can be influenced by rigid perineum (passage), inadequate uterine contractions (power), and a giant baby (passenger) which can lead to prolonged labor. Implications for midwifery, midwives can provide delivery process preparation by regarding childbirth complications early through the Darul Quran Sehat’s program. This study did not examine the strong relationship between Darul Quran Sehat’s program implementation and continuity delivery at first active phase so that further research is expected to investigate this matter.
