Abstract

Ulkus diabetik merupakan salah satu komplikasi kronik diabetes mellitus berupa luka terbuka pada permukaan kulit yang dapat disertai adanya kematian jaringan setempat. Terapi perawatan ulkus diabetik yang adekuat diharapkan dapat mempercepat proses penyembuhan secara optimal. Cacing tanah (Pheretima aspergillum) memiliki komponen bioaktif EGF (Epidermal Growth Factor) yang memfasilitasi regenerasi sel epidermal dan memainkan peran penting dalam proses penyembuhan luka dermal melalui stimulasi poliferasi, migrasi keratosit, pembentukan jaringan granulasi dan motilitas fibroblas. Penelitian ini bertujuan untuk mengetahui pengaruh ekstrak cacing tanah terhadap peningkatan kontraksi luka pada tikus yang diinduksi ulkus diabetik derajat II. Penelitian ini menggunakan desain true-experimental posttest only controlled group. Sampel dibagi ke dalam lima kelompok: NaCl 0,9% sebagai kontrol negatif, Hidrogel sebagai kontrol positif, dan tiga kelompok perlakuan mendapat ekstrak cacing tanah secara topikal, oral, dan topikal-oral. Variabel yang diteliti adalah peningkatan kontraksi luka (dihitung pada hari ke-21 rawat luka). Hasil penelitian menunjukkan kelompok perlakuan yang mendapatkan ekstrak cacing tanah secara bermakna memiliki kontraksi luka lebih tinggi dibandingkan dengan kelompok kontrol (p=0.013). Pemberian ekstrak cacing tanah secara topikal-oral memiliki efek optimal terhadap peningkatan kontraksi luka. Dapat disimpulkan bahwa ekstrak cacing tanah (Pheretima aspergillum) dapat meningkatkan kontraksi luka pada tikus model diabetik.

English Abstract

Diabetic ulcer is one of the chronic complication of diabetes mellitus in the form of an open wound on the skin surface which can be accompanied by local tissue death. Adequate treatment of diabetic ulcer is expected to accelerate the healing process optimally. Earthworm (Pheretima aspergillum) has bioactive component of EGF (Epidermal Growth Factor) which facilitates regeneration of epidermal cells and plays an important role in dermal wound healing process through stimulation of proliferation, migration of keratinocytes, formation of granulation tissue and fibroblasts motility. This study aimed to determine the effect of earthworm extract on the increase of wound contraction in rats induced diabetic ulcer stage II. This study used a true-experimental posttest only controlled group design. Samples were divided into five groups: 0.9 % NaCl as a negative control group, Hydrogel as positive control group, and the three treatment groups received earthworm extract via topical, oral, and topical-oral. The variable studied were the increase of wound contraction (calculated on day 21 wound care). The results showed that the treatment groups received earthworm extract had significantly higher wound contraction compared to the control groups (p=0.013). The administered of earthworm extract via topical-oral has an optimal effect on the increase of wound contraction. It can be concluded that the extract of earthworm (Pheretima aspergillum) may improve wound contraction in rats model of diabetic.
