Abstract
Bakteri MRSA diakui sebagai bakteri patogen penyebab infeksi nosokomial di seluruh dunia. Bakteri ini mampu berkolonisasi di kulit, hidung, dan tenggorok dengan penyebaran melalui kontak langsung maupun tidak langsung sehingga berperan dalam terjadinya infeksi MRSA di rumah sakit. Penyebaran bakteri MRSA dapat dicegah dengan tindakan cuci tangan oleh petugas kesehatan sesuai dengan standar WHO. Penelitian ini merupakan penelitian kuasi dengan desain pretest dan posttest yang dilakukan dalam tiga fase yaitu pre intervensi, intervensi dan post intervensi. Pada fase pre dan post intervensi dilakukan pengamatan kepatuhan cuci tangan oleh petugas kesehatan serta skrining untuk menentukan angka karier MRSA pada penderita. Pada fase intervensi dilakukan tindakan intervensi berupa edukasi cuci tangan dan pemasangan handrub alkohol-gliserin dengan kandungan klorheksidin 0,5% pada setiap bed pasien. Data dianalisis dengan uji crossab . Analisis statistika menunjukkan peningkatan yang bermakna tingkat kepatuhan cuci tangan sesuai standar WHO oleh petugas kesehatan setelah tindakan intervensi yaitu sebesar 64% dibandingkan dengan fase pre intervensi yaitu sebesar 18% (p<0,001). Selain itu diperoleh penurunan bermakna angka karier MRSA yang mulanya sebesar 11% pada fase pre intervensi menjadi 3% pada fase post intervensi (p<0,001). Kesimpulan dari penelitian ini adalah peningkatan kepatuhan cuci tangan petugas kesehatan sesuai standar WHO diikuti dengan penurunan angka karier MRSA pasien bedah di RSUD Dr. Saiful Anwar Malang.
English Abstract
Methiciliin Resistant Staphylococcus aureus (MRSA) is recognized as pathogen causing nosocomial infections worldwide. The bacteria colonize on the skin, nose, and throat with the spread through direct contact or indirectly to play a role of MRSA infections in hospitals. The spread of MRSA can be prevented by hand hygiene by health care workers in accordance with WHO standards. This study, a quasi experimental with pretest and posttest design, was carried out in three phases: pre-intervention, intervention and post-intervention. In the pre and post intervention phases, hand hygiene compliance by health care workers were observed as well as screening to determine the carriage rate of MRSA among patients. In the intervention phase, we performed intervention measure with hand hygiene education and set up alcohol-glycerine handrub containing chlorhexidine 0.5 % on each patient bed. Data were analyzed with chi square test. Statistical analysis showed a significant improvement in hand hygiene compliance rate according to WHO standards by health care workers after the intervention in the amount of 64 % compared to pre-intervention phase that is 18% ( p <0,001), whereas the MRSA carriage rate was decrease from 11% in the pre-intervention phase to 3 % in the post-intervention phase ( p <0,001). It is concluded that the improvement of hand hygiene compliance according to WHO standards of health care workers is followed by a decrease of MRSA carriage rate among patients in surgical ward Dr Saiful Anwar Malang Hospital.
