Abstract

Hiperglikemia kronik pada diabetes mellitus berhubungan dengan kerusakan jangka panjang, disfungsi, dan kegagalan berbagai organ dalam tubuh terutama jantung. Salah satu komplikasi diabetes pada jantung yaitu diabetes kardiomiopati yang merupakan penyebab utama mortalitas dan morbiditas pada pasien diabetes. Diabetes kardiomiopati merupakan kondisi yang mempengaruhi miokardium. Asam alfa lipoat efektif terhadap berbagai kondisi patologis yang disebabkan oleh ROS (Reactive Oxygent Species). Penelitian ini bertujuan untuk menginvestigasi efek ALA terhadap stres oksidatif pada jantung tikus Wistar jantan yang diinduksi diabetes mellitus. Rancangan penelitian True experimental design dan Posttest Only Control Group digunakan dalam penelitian ini. Tiga puluh ekor tikus (Rattus norvegicus galur wistar) dibagi menjadi lima kelompok secara acak yaitu: kelompok tikus normal tanpa perlakuan ALA (NTA), tikus diabetes tanpa perlakuan ALA (DTA), tikus diabetes dengan ALA dosis 80 mg/kg/hari (DA80), tikus diabetes dengan ALA dosis 200 mg/kg/hari (DA200), dan tikus diabetes dengan ALA dosis 500 mg/kg/hari. Induksi diabetes pada tikus dilakukan intraperitonial dosis tunggal streptozotocin 60 mg/kg berat badan. Kadar malondialdehid (MDA) pada miokardium diukur melalui metode spektrofotometri. Struktur jantung (ventrikel dan septum) diuji setelah diwarnai dengan hematoksilin dan eosin. Tingkat mortalitas DA500 lebih tinggi dibandingkan kelompok NTA (p = 0,014), DTA (p = 0,002), DA80 (p = 0,344), dan DA200 (p = 0,344). Kelompok DA80 dan DA200 menunjukkan peningkatan berat badan, penurunan gula darah puasa, dan kadar MDA, tetapi tidak signifikan secara statistik pada berat badan (p = 0,392), gula darah puasa (p = 0,368), dan MDA (0,451). Rasio berat jantung terhadap berat badan pada DA80 dan DA200 tidak berbeda signifikan dengan NTA, DTA, dan DA500 (p = 0,588). Tidak ada perbedaan struktur miokardium diantara NTA dan DA80. Kesimpulan dari penelitian ini, pemberian ALA selama 4 minggu belum menurunkan stres oksidatif pada jantung tikus diabetes.

English Abstract

The chronic hyperglycemia of diabetes is associated with long term damage, dysfunction, and failure of various organs in the body, especially heart. Cardiovascular complications, including diabetic cardiomiopathy are major causes of morbidity and mortality in diabetic patients. Diabetic cardiomiopathy is a condition that affects the myocardium. Alpha lipoic acid is effective in many pathological conditions where ROS (Reactive Oxygent Species) have been implicated. This study was undertaken to investigate effects of ALA on oxidative stress in diabetic heart of male Wistar rats. True experimental design and Posttest Only Control Group are used in this study. Thirty male rats were randomly divided into 5 groups: normal rats without ALA (NTA), diabetic rats without ALA (DTA), diabetic rats with ALA 80 mg/kg/day (DA80), diabetic rat with ALA 200 mg/kg/day (DA200), and diabetic rat with ALA 500 mg/kg/day (DA500). Diabetes was induced in rats by single intraperitonial injection of streptozotocin (STZ) at 60 mg/kg body weight. The content of malondialdehyde (MDA) in the myocardium was measured by spectrophotometeric assays. Cardiac structure (ventricle and septum) was assessed by staining with hematoxylin and eosin. The total mortality rate was higher for the DA500 group compared with NTA (p = 0,014), DTA (p = 0,002), DA80 (p = 0,344) and DA200 (p = 0,344). The DA80 and DA200 groups exhibited increased body weight, reduced fasting blood glucose and MDA but did not achieve statistical significance for body weight (p = 0,392), fasting blood glucose (p = 0,368), and MDA (p = 0,451). The heart to body weight ratio in the DA80 and DA200 were not significantly different from NTA, DTA, and DA500 group (p = 0,588). No structural difference was found between NTA and DA80 on myocardium. In conclusion, the findings of the present experimental study indicate that treatment for 4 weeks with ALA had not reduced oxidative stress in diabetic heart.
