Abstract

Latar balakang: Salah satu penanganan utama pada pasien tukak diabetik adalah Antibiotik. Antibiotik harus digunakan secara tepat karena penggunaannya yang tidak efektif dan efisien mendorong berkembangnya resistensi antibiotik. Tujuan: Penelitian ini ditujukan untuk mengetahui pola penggunaan antibiotik pada pasien tukak diabetik berdasarkan derajat keparahan luka IDSA 2012 di RSUD dr Saiful Anwar Malang. Metode: Penelitian dilaksanakan dengan cara pengambilan data secara langsung kemudian dianalisis deskriptif. Hasil: Didapatkan 38 pasien tukak diabetik. Antibiotik empiris yang paling banyak digunakan pada pasien dengan tukak diabetik adalah Metronidazole (86,84 %), kemudian Cipofloxacin (55,26 %), Ceftriaxone (52,63 %), dan Clindamycin (36,84 %). Dari pemilihan antibiotik tersebut hanya 13,16 % terapi antibiotik yang sesuai dengan IDSA guideline. Hal ini bisa dikarenakan tidak adanya suatu pedoman lokal antibiotik untuk pasien tukak diabetik khususnya di RSSA. Kesimpulan Penggunaan antibiotik empiris pada pasien tukak diabetik belum sesuai dengan IDSA Guideline. Oleh karena itu diperlukan suatu pedoman lokal yang disesuaikan dengan pola bakteri di rumah sakit yang bersangkutan untuk meningkatkan efisiensi dan efektifitas penggunaan antibiotik serta mencegah resistensi antibiotik.

English Abstract

Background: Antibiotic is the prominent therapy for diabetic foot ulcers patient. Antibiotics should be used appropriately because an inappropriate use of antibiotics may develop the antibiotic resistance. Objective: This study aimed to determine the pattern of antibiotic usage in patients ith diabetic ulcers based on the injury severity degree by IDSA guidelines 2012 at Dr. Saiful Anwar Hospital Malang. Method: The research was conducted by collecting data directly and then analyzed descriptively. Results: This study found 38 patients of diabetic foot ulcers. Empirical antibiotic which were used in patients with diabetic ulcers were Metronidazole (86.84%), Ciprofloxacin (55.26%), Ceftriaxone (52.63%), and Clindamycin (36.84%). From the selection of antibiotics in RSSA, there was only 13.16% appropiate antibiotic therapy according to the IDSA guidelines. This could be due to the lack of a local antibiotic guideline for diabetic ulcer patients especially in RSSA. Conclusion: The use of empirical antibiotics in patients with diabetic ulcers had not been in accordance with the IDSA Guidelines. Therefore local guideline is required to improve the efficiency and effectiveness of the use of antibiotics and prevent antibiotic resistance.
