Indonesian Abstract

Diabetes melitus tipe 2 memiliki karakteristik meningkatnya kadar glukosa darah (hiperglikemia). Hal terjadi karena terganggunya fungsi sel beta pankreas yang mengakibatkan berkurangnya produksi insulin sehingga menimbulkan keadaan hiperglikemia. Ekstrak etanol daun tembakau (Nicotiana tabacum) yang terkandung nikotin dosis rendah dapat menurunkan kadar glukosa darah dengan menstimulasi sel beta pankreas untuk meningkatkan sekresi insulin dan menghambat inflamasi di jaringan adiposa yang menghambat resistensi insulin. Nikotin menstimulasi sel beta pankreas untuk mensekresi insulin melalui aktivasi nAChR, aktivasi nAChR meningkatkan sensitivitas kalsium pada sel tersebut. Peningkatan intraseluler dari kalsium dapat menstimulasi sekresi insulin pada sel beta pankreas. Penelitian ini bertujuan untuk membuktikan pengaruh pemberian ekstrak etanol daun tembakau dalam menurunkan kadar glukosa darah pada tikus putih wistar jantan (Rattus norvegicus) model diabetes melitus tipe 2. Desain penelitian menggunakan metode eksperimental murni secara in vivo dengan rancangan randomized post test only controlled group design. Penelitian ini memiliki 25 sampel yang dibagi menjadi 5 kelompok yaitu kelompok kontrol negatif, kontrol positif, dan 3 kelompok perlakuan yang masing-masing diberikan ekstrak etanol daun tembakau secara oral dengan dosis 90 mg/KgBB, 180 mg/KgBB, dan 270 mg/KgBB selama 28 hari. Kadar glukosa darah diukur setelah diinduksi dan setiap 7 hari sekali setelah tikus dinyatakan diabetes melitus tipe 2. Analisis data menggunakan One-Way ANOVA, Post Hoc LSD, dan Regresi Linier. Pada uji One-Way ANOVA di dapatkan nilai signifikansi 0.000 (p<0.05), hal ini dapat membuktikan bahwa adanya perbedaan bermakna kadar glukosa darah antara kelompok kontrol negatif, kontrol positif (diinduksi diabetes melitus tipe 2), dan kelompok perlakuan (diberikan ekstrak etanol daun tembakau). Berdasarkan hasil penelitian ini, dapat disimpulkan bahwa ekstrak etanol daun tembakau (Nicotiana tabacum) memiliki pengaruh dalam menurunkan kadar glukosa darah pada tikus wistar jantan model diabetes melitus tipe 2.

English Abstract

Diabetes mellitus type 2 is a group of metabolic diseases that have characteristics of increased blood glucose levels (hyperglycemia). This happens cause of disruption of pancreatic beta cell function resulting in reduced production of insulin, causing a state of hyperglycemia. Decreased peripheral tissue response to insulin or insulin resistance and decreased ability of pancreatic beta cells to secrete insulin in response to glucose loading leads to type 2 diabetes mellitus. Ethanol extract of tobacco leaves (Nicotiana tabacum) containing low doses of nicotine can lower blood glucose levels by stimulating pancreatic beta cells to increase insulin secretion and inhibit inflammation in adipose tissue that inhibits insulin resistance. Nicotine stimulates pancreatic beta cells to secrete insulin through activation of nAChR, activation of nAChR increases calcium sensitivity in these cells. The intracellular increase of calcium may stimulate insulin secretion in pancreatic beta cells.This study aims to prove the effect of ethanol extract of tobacco leaves in reducing blood glucose levels in male wistar white rats (Rattus norvegicus) type 2 diabetes mellitus model. The study was designed used true experimental method in vivo with randomized post test only controlled group design. This research has 25 samples divided into 5 groups: negative control group, positive control, and 3 treatment groups each given oral ethanol extract of oral leaf with dose of 90 mg/Kg Body Weight, 180 mg/Kg Body Weight, and 270 mg/Kg Body Weight for 28 days. Blood glucose levels were measured after induced and once every 7 days after mice were declared type 2 diabetes mellitus. Data analysis used One-Way ANOVA, Post Hoc LSD, and Linear Regression. In the One-Way ANOVA test, the significance level of 0.000 (p <0.05) showed that significant differences in blood glucose levels between negative control group, positive control (induced type 2 diabetes mellitus) and treatment group (ethanol extract tobacco leaf). The conclusions of this study that prove the ethanol extract of tobacco leaves (Nicotiana tabacum) has an effect in reducing blood glucose levels in male wistar rats model T2DM.
