Abstract

Cedera kepala merupakan suatu kondisi terjadinya trauma pada kepala yang dapat mengakibatkan kerusakan jaringan otak akibat adanya trauma. Di Indonesia kejadian cedera kepala setiap tahunnya diperkirakan mencapai 500.000 kasus. Dari jumlah tersebut, 10% penderita meninggal sebelum tiba di rumah sakit. Dari pasien yang sampai di rumah sakit, 80% dikelompokan sebagai cedera kepala ringan, 10 % termasuk cedera kepala sedang, dan 10 % termasuk cedera kepala berat. Cedera kepala berat memiliki tingkat mortalitas yang tinggi, oleh karena itu mengetahui prognosis cedera kepala berat menjadi sangat penting untuk memberikan informasi mengenai perjalanan penyakit. Beberapa faktor yang berhubungan prognosis cedera kepala berat adalah usia, jenis kelamin, nilai RTS dan nadi. Faktor yang mempengaruhi prognosis cedera kepala berat adalah lama prehospital, mekanisme cedera, transportasi dan trauma organ lain. Desain yang digunakan dalam penelitian ini adalah analitik observasional. Jumlah sampel dalam penelitian ini adalah 80 rekam medis yang memenuhi kriteria inklusi dan ekslusi yang didapatkan pada tanggal 25 Mei sampai 6 Juni 2016. Analisa data menggunakan koefisien kontingensi dan spearman serta analis multivariat menggunakan regresi logistik. Alat ukur yang digunakan adalah lembar observasi. Hasil penelitian menunjukkan bahwa terdapat hubungan antara usia (p=0,009) nlai RTS (p=0,000), transportasi(p=0,013) dan trauma organ lain (p=0,000). Tidak ada hubungan antara jenis kelamin (p=0,783), lama prehospital (p=0,180), frekuensi nadi (p=0,221), mekanisme cedera (p=0,625). Hasil uji regresi logistik didapatkan bahwa trauma organ lain (p=0,00; RR=9,389) merupakan faktor dominan yang mempengaruhi prognosis pasien cedera kepala berat. Kesimpulan penelitian ini adalah ada hubungan antara faktor usia,nilai RTS, transportasi dan trauma organ lain terhadap prognosis pasien cedera kepala berat. Jenis kelamin, lama prehospital, frekuensi nadi dan mekanisme cedera tidak memliki hubungan terhadap prognosis cedera kepala berat. Trauma pada organ lain merupakan faktor dominan yang memiliki hubungan terhadap prognosis pasien cedera kepala berat. Keterbatasan dalam penelitian ini adalah peneliti tidak meneliti tentang bagaimana mekanisme cedera yang lebih spesifik dan itu peneliti tidak meneliti apakah tindakan pertolongan pertama diberikan kepada korban cedera kepala berat.

English Abstract

Head injury is a condition of trauma in the head that can made brain tissue damage due to trauma. In Indonesia, the incidence of head injury every year is estimated to reach 500,000 cases. Unfortunately 10% of the data, patients die before arriving at the hospital. Based on the patients arriving at the hospital, 80% were classified as mild head injury, 10% moderate head injury, and 10% severe head injury. Severe head injuries showed a high mortality rate, therefore understanding the prognosis of severe head injury becomes very important to provide information on the disease history. Some of the factors that related the prognosis of severe head injury are age, gender, RTS value, pulse rate. Some of factors that affect the prognosis of severe head injury are pre-hospital length, injury mechanism, transportation and other traumatic organs. The design applied in the study was observational analytics. The number of samples taken were 80 medical records which fitted to the inclusion and exclusion criteria, it was obtained from May 25th - June 6th, 2016. Contingency coefficient and spearman and multivariate analysis through logistic regression were applied to analyze the data. The instrument applied was the observation sheet. The findings showed that there were correlations among age (p = 0,009) RTS value (p = 0,000), transportation (p = 0,013) and other traumatic organs (p = 0,000). And there were no correlation among sex (p = 0,783), pre-hospital length (p = 0,180), pulse rate (p = 0,221), injury mechanism (p = 0,625). Logistic regression test discovered that other traumatic organs (p = 0,00, RR = 9.389) were the dominant factor associated with the prognosis of severe head injury patients. It could be drawn that there was a correlation among factors of age, RTS value, transportation and other traumatic organs to the prognosis of severe head injury patients. However sex, pre-hospital length, pulse rate and injury mechanisms were considered not to have correlation to the prognosis of severe head injury. Trauma to other organs was a dominant factor that had a correlation to the prognosis of severe head injury patients. Limitation of the study was that the researchers did not measure how the mechanism of injury more specificly and first aid kid given to the severe head injury patients.
