Indonesian Abstract

Sepsis merupakan suatu sindroma klinis dimana terdapat disfungsi organ yang mengancam jiwa akibat respon tubuh yang tidak sesuai terhadap infeksi. Pada sepsis terjadi inflamasi sistemik yang tidak teratur yang mengakibatkan naiknya sitokin proinflamasi dan memicu produksi Prokalsitonin. Akan tetapi, bagaimanakah hubungan antara Prokalsitonin dengan mortalitas pada pasien sepsis belum banyak diketahui. Tujuan penelitian ini untuk menganalisa hubungan antara kadar Prokalsitonin dengan mortalitas penderita sepsis yang dirawat di Rumah Sakit Saiful Anwar Malang. Penelitian ini menggunakan metode observasional analitik dengan pendekatan cohort prospektif. Subyek penelitian sebanyak 32 pasien yang diambil dengan teknik consecutive sampling. Hasil uji independent T-test didapatkan perbedaan kadar Prokalsitonin yang bermakna berdasarkan mortalitas pada pasien sepsis dengan signifikansi p = 0,000 (∝0,05). Hasil uji korelasi Pearson menunjukkan terdapat hubungan positif sedang antara kadar Prokalsitonin dengan mortalitas penderita sepsis (p = 0,000, r = 0,654). Berdasarkan analisis Relative Risk (RR) Prokalsitonin dengan cut-off 9,725 U/L (AUC = 89,8%, sensitivitas = 94,1% dan spesifisitas = 86,7%), pasien dengan ≥ 9,725 U/L memiliki RR = 12,44 kali mengalami kematian (95% CI : 2,244 – 31,119). Kesimpulannya adalah terdapat hubungan positif yang sedang antara kadar prokalsitonin dengan mortalitas penderita sepsis.

English Abstract

Sepsis is a clinical syndrome where there is life-threatening organ dysfunction due to an inappropriate body response to infection. In sepsis there is an irregular systemic inflammation which results in an increase in proinflammatory cytokines and triggers Procalcitonin production. However, how is the relationship between Procalcitonin and mortality in sepsis patients not yet known. The purpose of this study was to analyze the relationship between levels of Procalcitonin and the mortality of patients with sepsis who were treated at Saiful Anwar Hospital Malang. This study uses an observational analytic method with a prospective cohort approach. Research subjects were 32 patients who were taken by consecutive sampling technique. The independent T-test results obtained significant differences in the levels of Procalcitonin according to patient’s mortality with a significance of p = 0,000 (∝0.05). Pearson correlation test results showed a moderate positive relationship between levels of Procalcitonin with the mortality of patients with sepsis (p = 0,000, r = 0.654). Based on the analysis of Relative Risk (RR) of Procalcitonin with a cut-off of 9,725 U / L (AUC = 89.8%, sensitivity = 94.1% and specificity = 86.7%), patients with ≥ 9,725 U / L have RR = 12 , 44 times experienced death (95% CI: 2,244 - 31,119). The conclusion is that there is a moderate positive relationship between procalcitonin levels and mortality in patients with sepsis.
