Indonesian Abstract

Kanker paru merupakan penyebab kematian tersering pada kasus kanker. Paparan berbagai faktor eksogen dan endogen menyebabkan kerusakan genetik yang terjadi selama proses replikasi DNA. MSH3 merupakan salah satu gen yang termasuk dalam sistem mismatch repair yang mampu mengenali mismatch basa atau insertion/deletion loop selama proses replikasi DNA. MSH3 rs26279 dianggap terlibat dalam karsinogenesis serta berkaitan dengan respon kemoterapi golongan platinumoleh karena dapat menghilangkan platinum-DNA interstrand crosslink sehingga menyebabkan penurunan respon kemoterapi. Penelitian ini bertujuan mengetahui peranan polimorfisme MSH3 rs26279 terhadap respon progresif kemoterapi berbasis platinum lini I. Metode: Penelitian case control yang melibatkan 20 orang pasien KPKBSK yang terdiri dari 10 orang pasien dengan respon kemoterapi progresif dan 10 pasien dengan respon non progresif. Pemeriksaan polimorfisme MSH3 menggunakan automated sanger sequencing. Hasil: Polimorfisme MSH3 rs26279 genotip A/A lebih berkaitan dengan respon kemoterapi progresif (OR: 13,5; 95% CI: 1,197-152,211; p=0.029) sedangkan genotip G/G lebih memiliki efek proteksi terhadap respon progresif (OR: 0,167; 95% CI: 0,23-1,232; p=0.085). Genotip G/A juga memiliki efek proteksi terjadinya respon progresif namun rationya lebih kecil dibanding G/G. Kesimpulan: Polimorfisme MSH3 rs26279 terbukti berperan terhadap respon progresif kemoterapi berbasis platinum pada pasien KPKBSK dimana respon progresif lebih berkaitan dengan genotip A/A.

English Abstract

Lung cancer is the most common cause of death related cancer. Exposure of various exogenous and endogenous factors causes genetic defects that occur during DNA replication process. MSH3 is one of the mismatch repair genes that are able to recognize base mismatch or insertion/deletion loop during DNA replication. MSH3 rs26279 is considered to be involved in carcinogenesis and associated with platinum group chemotherapeutic response, because it can eliminate platinum-DNA interstrand crosslinks, leading to decreased chemotherapy response.The aim of this study to determine the role of polymorphisms MSH3 rs26279 on the progressive response of first line platinum-based chemotherapy. Methods: case-control study involving 20 patients NSCLC consisting of 10 patients with progressive response to chemotherapy and 10 patients with the non-progressive response. Examination of MSH3 polymorphism using Automated Sanger sequencing. Results: MSH3 rs26279 polymorphism A/A genotype is associated with progressive response of chemotherapy (OR: 13.5; 95% CI: 1,197-152,211; p=0.029), while the genotype G/G have more protective effects against progressive response (OR: 0,167; 95% CI: : 0,23-1,232; p=0.085). The G/A genotype also has a protective effect of progressive response but has smaller ratio than G/G. Conclusion: MSH3 rs26279 polymorphisms have been shown play a role in platinum-based chemotherapy response in NSCLC patients where the progressive response is more related to A/A genotype.
