Indonesian Abstract

Latar belakang: Obesitas sentral banyak didapatkan pada pria, dikenal sebagai “ android obesity/apple-shaped obesity ”. Jaringan lemak viseral berperanan terjadinya resistensi insulin dideteksi kuantitatif dengan HOMA-IR. USG sebagai modalitas pilihan untuk menilai distribusi lemak tubuh karena keunggulannya. Tujuan penelitian: Mengetahui korelasi rasio tebal lemak viseral dan subkutan dengan kadar HOMA-IR serta parameter SM (BMI, WC, GDP, TG dan HDLc) pada pria dewasa dengan obesitas sentral di RSU dr. Saiful Anwar Malang. Metode penelitian: Analytics Cross Sectional Survey di IRJ Penyakit Dalam Pria dan ruang USG RSU dr. Saiful Anwar serta laboratorium Faal FKUB Malang, September sampai dengan Desember 2011. Digunakan uji korelasi Spearman dan Oneway ANOVA serta uji alternatif Chi Square, yaitu Kolmogorov Smirnov. Semua perhitungan dianggap bermakna jika p 0,05. Hasil: Dari 37 sampel yang memenuhi kriteria inklusi, tidak didapatkan hubungan signifikan antara Abdominal wall fat index dan Rasio viseral/subkutan dengan HOMA-IR; Abdominal wall fat index dan Rasio viseral/subkutan dengan parameter SM, serta HOMA-IR dengan parameter SM (HDL dan TG), namun didapatkan hubungan signifikan antara HOMA-IR dengan parameter SM (BMI, WC, GDP dan Insulin) (p 0,05). Terdapat kecenderungan semakin tinggi derajat obesitas menurut BMI dan WC semakin meningkat rasio lemak viseral/subkutan dengan rerata ≥2,5 cm dan rerata HOMA-IR ≥4 ( glucosa molar units mmol/L ). Kesimpulan: Secara analisa deskriptif, terdapat kecenderungan peningkatan rasio lemak viseral/subkutan ≥2,5 dan HOMA-IR ≥4 ( glucosa molar units mmol/L ) sehingga dapat dipertimbangkan sebagai cut off point .

English Abstract

Background: Central obesity or “android obesity/apple-shaped obesity” mostly happen in men. Visceral fat tissue plays a role in insulin resistance which detected quantitatively by HOMA-IR. Ultrasound is modality of choice to assess body fat distribution because of its safety. Objective: To analyze correlation of visceral/subcutaneous fat ratio with HOMA-IR levels and parameters of MS (BMI, WC, FBG, TG and HDLc) in centrally obese men in RSU dr. Saiful Anwar Malang. Methods: This Analytics Cross Sectional Survey study took place in ambulatory unit and USG room of RSU dr. Saiful Anwar Malang from 1 September 2011 until 31 December 2011. Spearman`s bivariate correlation test, Oneway ANOVA, and Chi Square test alternatives; Kolmogorov Smirnov, were used to analyze correlation and comparation among variables. All statistics are significant if p 0.05. Results: Of 37 samples that fulfilled inclusion criteria, re are no significant relationship between abdominal wall fat index, and visceral/subcutaneous fat ratio with HOMA-IR (p=0.373;0.972); and also with MS parameters, and between HOMA-IR with SM Parameters (HDL and TG), but re are significant relationship between HOMA-IR with MS parameters (BMI, WC, FBG and insulin) (p 0.05). re are tendency to increase of obesity degrees that is followed by increasement of visceral/subcutaneous fat ratio ≥2.5 and HOMA-IR ≥4. Conclusion: visceral/subcutaneous fat ratio ≥2.5 and HOMA-IR ≥4 (molar glucose units mmol/L), can be considered as cut off point.
