Abstract
Tuberkulosis (TB) adalah penyakit menular yang disebabkan oleh Mycobacterium tuberculosis (Mtb). Di Indonesia terdapat 450.000 kasus baru/tahunnya dan 64.000 kematian/tahun dikarenakan tuberkulosis. Untuk mencegah penyakit TB, pemerintah telah melaksanakan program vaksin TB menggunakan BCG, namun pemberian vaksin BCG masih dipertanyakan keefektifannya. Protein 38 kDa M. tuberculosis adalah antigen spesifik M. tuberculosis yang paling dominan dalam merangsang respons imun tubuh dibanding antigen lainnya. Penelitian ini bertujuan untuk menentukan bahwa protein rekombinan 38 kDa M. tuberculosis dapat meningkatkan jumlah limfosit yang mengekspresikan IL-2 pada kultur PBMC kelompok pasien, kontak TB dan sehat. Metode yang digunakan adalah desain eksperimen murni di laboratorium secara ex vivo dengan Randomized Post Test Only Controlled Group Design. Sampel berjumlah masing-masing 8 subyek sehat, kontak dan pasien. PBMC dari masing-masing subyek diisolasi dan dibagi menjadi 3 kelompok yang kemudian diinduksi dengan protein 38 kDa, PPD dan kontrol negatif. PBMC akan dikultur selama 3 hari dan dilihat ekspresi IL-2 menggunakan Flowcytometry. Hasil penelitian menunjukkan rerata jumlah limfosit yang mengekpsresikan IL-2 pada kelompok sehat=0,66, kontak=0,6, dan pasien=1,74. Setelah dilakukan uji ANOVA, didapatkan perbedaan yang signifikan dari jumlah limfosit yang mengekspresikan IL-2 pada pemberian protein 38 kDa yang diinduksi pada masing-masing kelompok subyek sehat, kontak dan pasien, dengan nilai p sebesar 0,004. Sehingga kelompok pasien memiliki ekspresi IL-2 dari limfosit yang paling tinggi pada induksi protein 38 kDa M. tuberculosis. Sedangkan limfosit yang mengekspresikan IL-2 pada perlakuan pemberian protein 38 kDa Mtb, pemberian PPD, dan kontrol negatif tidak memberikan perbedaan yang signifikan baik pada kelompok sehat, kontak, maupun pasien. Kesimpulan penelitian adalah Protein M.tb 38kDa tidak dapat meningkatkan jumlah limfosit yang mengekspresikan IL-2 pada kultur PBMC.
English Abstract
Tuberculosis is an infectious disease that was caused by Mycobacterium tuberculosis (Mtb). In Indonesia there are 450,000 new cases/year and 64,000 deaths/year due to tuberculosis. To prevent TB, the government has implemented a program using BCG tuberculosis vaccine, but the effectiveness of BCG vaccine is still questionable. The 38 kDa Protein of Mycobacterium tuberculosis was the specific antigens of M.tuberculosis which most dominant in stimulating the immune response compared to other antigens. This study aimed to determine that the 38 kDa protein of M. tuberculosis can induce the level of lymphocytes that express IL-2 in PBMC culture of TB patient, contacts, and healthy volunteers. The method used was a purely experimental design in the laboratory ex vivo with Post Test Only Randomized Controlled Group Design. Each sample was from 8 healthy subjects, patients and contacts. PBMCs from each subject were isolated and divided into 3 groups were then induced with the recombinant 38 kDa protein, PPD and negative control. PBMCs were cultured for 3 days will be seen expression of IL - 2 using flowcytometry. The results showed the mean expression of IL - 2 lymphocytes (gate %) of healthy = 0.66, contacts = 0.6, and patients = 1.74. From ANOVA test, in healthy subjects, contacts and patients didn’t showed a significant difference to the production of IL - 2 in the induction of the 38 kDa protein Mtb, with p-value of 0.004. While lymphocytes that expressed IL-2 in the third group did not provide a significant difference. Conclusion of the study was the 38 kDa protein of M.tb can not increase the number of lymphocytes that express IL-2 in PBMC cultures.
